Orthodontic treatment with clear aligners in children

Abstract:

Nejmodernější metodou ortodontické terapie jsou v dnešní době fóliové aparáty, s dosavadním největším zastoupením produktu Invisalign, jež je často nazýván “neviditelnými rovnátky”. Tento typ léčby je často rychlejší a komfortnější alternativou pro pacienty s ortodontickými anomáliemi. Co je, ale v rámci naší práce stěžejní, je fakt, že ortodontická léčba zlepšuje polohu zubů a tím zjednodušuje jejich čištění, což je podstatné u každého pacienta, avšak u dětí to platí dvojnásob. Každá rovnátka, která pacienty motivují k zahájení ortodontické léčby, jsou proto pozitivem, přičemž Invisalign je díky svému komfortu a možnos-tem ideální variantou. Dále je pro dentální hygienistky podstatná také možnost pečovat o zuby v průběhu ortodontické léčby, což je výrazně snazší, rychlejší a efektivnější s fóliovými rovnátky na rozdíl od fixního aparátu. Cílem naší bakalářské práce bude podrobné představení této varianty léčby se zaměřením na dětské pacienty. V teoretické části představujeme nejčastější ortodontické anomálie ve smíšeném chrupu, možnosti jejich ortodontické léčby a druhy ortodontických aparátů. Doporučujeme vhodné pomůcky pro domácí dentální hygienu, zmiňujeme správnou péči o fóliové aparáty a v neposlední řadě se zaměřujeme na komplexní péči o dutinu ústní, která je nepostradatelnou součástí každé úspěšné ortodontické terapie. Součástí praktické části bakalářské práce je dotazníková studie zaměřovaná, jak na rodiče, tak na děti samotné. Hledáme odpovědi na otázky ohledně průběhu léčby s fóliovými aparáty, na schopnost spolupráce dětského pacienta, jeho pocity, radosti i strasti, které tento typ terapie přináší. Zaměřujeme se také na pohled rodiče, který s dítětem celou léčbou prochází a může tedy zhodnotit nejen její reálný průběh, omezení, problémy, spolupráci nebo motivaci dítěte, ale také změny v pohybech zubů a celkový výsledek. Jedním z důležitých cílů je také získat relevantní data, jako podklad pro motivaci budoucích malých pacientů a jejich rodičů. Ukázat, že tento typ terapie stojí za pozornost a nabízí nám mnoho možností, které konvenčním aparáty nemají, od možnosti brzkého zahájení léčby, přes efektivní pohyby zubů po výra. Léčbu s fóliovými rovnátky u dětí považujeme za průlom v ortodontické léčbě této věkové kategorie a proto budeme rádi, pokud naše práce poslouží k jejímu rozšíření a pomůže co nejvíce dětským pacientům - funkčně i esteticky a v našem případě i z pohledu usnadnění dentální hygieny a jejích správných a pravidelných návyků.

Abstract:

The most modern method of orthodontic therapy today are clear aligners, with the largest share of the Invisalign product to date, which is often called “invisible braces”. This type of treatment is often a faster and more comfortable alternative for patients with orthodontic anomalies. What is, but crucial in our work, is the fact that orthodontic treatment improves the position of the teeth and thus simplifies their cleaning, which is essential for every patient, but this is doubly true in children. Any braces that motivate patients to start orthodontic treatment are therefore positive, while Invisalign is an ideal option due to its com-fort and capabilities. Furthermore, the possibility of caring for teeth during orthodontic treatment is also essential for dental hygienists, which is significantly easier, faster and more effective with foil braces, unlike a fixed appliance. The aim of the bachelor thesis will be a detailed presentation of this treatment variant with a focus on pediatric patients. In the theoretical part we present the most common orthodontic anomalies in mixed teeth, the possibilities of their orthodontic treatment and types of orthodontic appliances. We recommend suitable aids for home dental hygiene, we mention the proper care of clear aligners and, last but not least, we focus on comprehensive care for the oral cavity, which is an indispensable part of any successful orthodontic therapy. Part of the practical part of the bachelor's thesis is a questionnaire study focused on both parents and children themselves. We are looking for answers to questions about the course of treatment with clear aligners, the ability of the child patient to cooperate, his feelings, joys and sorrows that this type of therapy brings. We also focus on the view of the parent who goes through the whole treatment with the child and can therefore evaluate not only its real course, limitations, problems, cooperation or motivation of the child, but also changes in tooth movements and the overall result. One of the important goals is also to obtain relevant data as a basis for motivating future young patients and their parents. To show that this type of therapy is worth paying attention to and offers us many options that conventional devices do not have, from the possibility of early treatment, through effective tooth movements to oozing. We consider treatment with clear aligners in children to be a breakthrough in orthodontic treatment in this age group, and therefore we will be happy if our work serves to expand it and help as many pediatric patients as possible - functionally and aesthetically and in our case facilitating den-tal hygiene and its proper and regular habits.
