Basic health screening in rural communities in Madagascar and recommendations for preventive measures

Abstract:

Úvod: Madagaskar má přibližně 29 milionů obyvatel, kdy více než 60 % z nich žije více než 5 km od nejbližšího zdravotnického střediska, a to ve velmi odlehlých oblastech bez cest a komunikací. Mezi nejčastějšími příčinami úmrtí jsou cévní mozková příhoda, infekce dolních dýchacích cest, ischemická choroba srdeční či tuberkulóza. Cílem práce je pilotní zdravotní screening obyvatel a prověření, zda se vyšetření dají v terénu realizovat. Metodika: Studie byla podpořena projektem SGS15/LF/2023 s názvem Základní screening zdravotního stavu ve vesnických komunitách na Madagaskaru a realizovaná v létě 2023. Jednalo se o screening vybraných parametrů zdravotního stavu a jejich rizikových faktorů u dobrovolníků v odlehlých oblastech Madagaskaru. Screening konkrétně zahrnoval sledování skladby jídelníčku, základní ukazatele civilizačních onemocnění (hodnoty glykémie, celkového cholesterolu a krevního tlaku, tělesných parametrů a test okultního krvácení do stolice) a testování infekčních nemocí (HIV, virová hepatitida typu B, virová hepatitida typu C, syfilis a tuberkulóza) celosvětového významu ve dvou komunitách (pobřežní a vnitrozemská). Data byla hodnocena v programu Stata 17, byla použita popisná statistika a statistické testy na hladině významnosti 5 % (chí-kvadrát test, Fisherův exaktní test, Mann-Whitney test) Výsledky: Do studie se zapojilo 256 osob, zastoupení osob v obou kohortách bylo rovnoměrné (47,27 % vnitrozemská, 52,73 % přímořská). Byly nalezeny signifikantní rozdíly ve skladbě jídelníčku, kdy na pobřeží byla častější konzumace ryb, luštěnin či sladkých jídel a méně častá konzumace ovoce a zeleniny. Nejčastější konzumovanou skupinou potravin byla zelenina. Byly nalezeny statisticky významně vyšší hladiny glykémie u osob starších 45 let a také u mužů oproti ženám. Signifikantní rozdíly byly nalezeny také u celkového cholesterolu, který byl častěji nad normou u přímořské kohorty a u žen oproti mužům. Hodnota krevního tlaku byla statisticky vyšší u starších 45 let. Také výskyt některých infekčních nemocí (HIV, VHC a syfilis) byl statisticky významně vyšší v přímořské komunitě. Závěr: Výsledky poukazují na horší zdravotní stav v přímořské komunitě oproti komunitě vnitrozemské, proto je zde vhodné cílit intervenci.

Abstract:

Introduction: Madagascar has appriximately 29 milion inhabitants, more than 60% of the rural population lives more than 5 kilometers from the nearest health centre, in very remote areas without roads or communications. The main causes of death are stroke, lower respiratory infectious and tuberculosis. The aim was to conduct a pilot health screening of the population and to test whether screening could be implemented in the field. Methodology: The study was supported by the SGS15/LF/2023 project entitled Basic Health Screening in Rural Communities in Madagascar and implemented in the summer of 2023. It involved screening selected health parameters and their risk factors in volunteers in remote areas of Madagascar. Specifically, the screening included monitoring composition of the diet, basic indicators of non-communicable diseases (hyperglycemia, hypercholesterolemia, hypertension, measurement of body parameters and fecal occult blood test) and testing for infectious diseases (HIV, hepatitis B, hepatitis C, syphilis and tuberculosis) of global importance in two communities (coastal and inland). Descriptive statistics, statistical tests (chi-square test, Fisher's exact test and Mann-Whitney test) at the significance level of 5 % were used to analysethe obtained results. Stata version 17 was used for data processing. Results: 256 people participated in the screening, with an even representation of people in both cohorts (47.27% inland, 52.73% coastal). Significant differences in diet composition were found. On the coast, the consumption of fish, legumes, sweet and fried foods was more frequent and the consumption of fruits and vegetables was less frequent. Vegetables were the most frequently consumed food group. Statistically significantly higher glycaemic levels were found in people over 45 years of age and also in men. Significant differences were also found for total cholesterol levels, where higher values were found in women and the coastal community. Blood pressure values were statistically higher in those over 45 years of age. Significant differences were also found for HIV, syphilis and hepatitis C, between cohorts. Conclusion: The results indicate poorer health status in the coastal community compared to the inland community, so it is appropriate to target the intervention here.
