Women's awareness of the risk of urinary incontinence after selected surgical deliveries

Abstract:

Inkontinence moči je intimní a choulostivý problém každé ženy, která tuto nepříjemnou situaci prožila nebo zrovna prožívá. Málokdo však ví, že důvodem, proč k úniku moči dochází, je prodělaný porod – ať už porod samotný nebo vedený pomocí kleští či vakuumextraktoru. Právě ten způsobuje oslabení svalů pánevního dna a s tím spojené riziko vzniku inkontinenci moči. V teoretické části bakalářské práce popisuji podstatu problému. Vysvětluji, co to inkontinence moči znamená a jaké typy medicína rozlišuje. Dále popisuji přirozený porod a operativní porod. U každého popsaného operativního porodu jsem uvedla podmínky pro provedení výkonu, indikace, kontraindikace a komplikace. Závěr teoretické části jsem věnovala anatomii a poruchám pánevního dna a zároveň jsem popsala, jaké dopady na něj má vliv těhotenství a porod. Pro praktickou část jsem použila kvalitativní metodu. Prostřednictvím anonymního polostrukturovaného rozhovoru jsem zkoumala informovanost žen o inkontinenci moči po vybraných operativních porodech. Zajímalo mě, zda ženy o této problematice vědí, zda byly informovány lékařem, a také jsem zjišťovala, zda za nimi dochází na poporodní oddělení fyzioterapeuti a učí je cviky na posílení pánevního dna. V bakalářské práci jsem došla k výsledku, že ženy po porodu pomocí vakuumextrakce nebo kleští sice ví, co je to inkontinence moči, ale ani jednu ženu o této problematice neinformoval její lékař. Pouze tři ženy z deseti byly o inkontinenci a oslabeném svalstvu pánevního dna informovány svojí porodní asistentkou. Z mého výzkumu zároveň vyplývá, že za všemi těmito ženami po porodu dochází na poporodní oddělení fyzioterapeutka, a poučuje je o správných cvicích na posílení pánevního dna, kterými ženy předchází inkontinenci moči.

Abstract:

Urine incontinence is an intimate and delicate problem of every woman who has experienced or is experiencing this unpleasant situation. However, just a few people know that the reason for a leakage of urine is childbirth – whether the natural one without tools or another one performed with forceps or vacuum extractors. It is the second one mentioned which causes the weakening of the pelvic floor muscles and appearing of the risk of urinary incontinence. In the theoretical part of the bachelor thesis I describe the essence of the problem. I explain what urinary incontinence means and what types are distinguished by medicine. Further, I describe natural birth and operative delivery. For each operative delivery I described conditions for performance, indications, contraindications and complications. In the conclusion of the theoretical part I focused on anatomy and pelvic floor muscles disorders and I also described the effects of pregnancy and childbirth on those muscles. For the practical part, I used a qualitative method. Through anonymous semi-structured interviews, I researched into women's awareness of urinary incontinence after selected operative deliveries. I took interest in whether women know about this issue, whether they were informed by a doctor, and I also asked whether physiotherapists visit them in the postpartum department and teach them how to exercise for strengthening the pelvic floor muscles. In the bachelor thesis I came to the conclusion that women after the birth by vacuum extractors or forceps know what urinary incontinence means, but not even one woman was informed about this issue by her doctor. Only three women out of ten were informed about the urinary incontinence and weakening of pelvic floor muscles by their midwife. My research also shows that physiotherapists from postpartum departments visit all these women after birth and teach them appropriate exercises for strengthening of pelvic floor muscles which prevent women from urinary incontinence.
