Diagnosticko-prognostický význam biomarkeru human epididymis protein 4 u karcinomu endometria

Diagnostic and prognostic value of human epididymis protein 4 in endometrial cancer

Abstract:

Úvod. Karcinom endometria je nejčastější gynekologická malignita v populaci českých žen. Přes poměrně příznivou prognózu se i v této oblasti výzkum zaměřuje na detekci markeru pro screening a včasnou diagnózu, z něhož by mohly profitovat zejména ženy se zvýšeným rizikem. U pacientek s high risk diagnostikovanou malignitou je provedení optimálního chirurgického stagingu často limitováno věkem a interními komorbiditami, většinou v kombinaci s různým stupněm obezity. Předmětem zájmu z tohoto pohledu by mohly být markery, které by mohly pomoci v diferenciaci prognosticky odlišných skupin pacientek a v individualizaci léčebného postupu. Metody. Do naší studie bylo v období červenec 2009 - červen 2014 zařazeno 115 pacientek s endometroidním adenokarcinomem endometria a 106 pacientek v kontrolním souboru s benigní lézí endometria. Histologický nález byl získán vyšetřením biopsie endometria. U všech pacientek byla po bioptické verifikaci stanovena sérová hladina lidského epididymálního proteinu 4 a konvenčního markeru CA125. Všechny pacientky s malignitou byly indikovány k operačnímu řešení. Medián a rozsahy sérových hladin obou biomarkerů byly analyzovány ve vztahu k biologické povaze nálezu a v souboru malignit ve vztahu k jednotlivým prognostickým faktorům. Statistická analýza pracuje s logaritmovanými hodnotami stanovených markerů vzhledem k distribuci sérových hladin HE4 a CA125 v obou souborech a využívá logistických regresních modelů. Výsledky. Při analýze dvou souborů biologicky odlišné povahy byl prokázán statisticky signifikantní rozdíl (p < 0,05) pouze v případě HE4, při cut-off 48,5 pmol/l bylo dosaženo senzitivity 87,8%, specificity 56,6% a AUC 0,773. Při statistické analýze souboru malignit je z hlediska klinické praxe nejvýznamnější průkaz statistické významnosti (p < 0,05) v případě HE4 pro diferenciaci pacientek s nízkým a vysokým rizikem. U modelu dosažení maxima součtu senzitivity a specificity vykazuje cut-off HE4 76,5 pmol/l pro diferenciaci těchto dvou skupin senzitivitu 72,4%, specificitu 75,4% a AUC 0,77 a prokazuje lepší diferenciační schopnost ve srovnání s CA125. Závěr. Diagnostický přínos biomarkeru HE4 by mohl být zvažován v součinnosti s klinickým a ultrazvukovým nálezem zejména v kohortě pacientek se zvýšeným rizikem karcinomu endometria a u pacientek výrazně interně kompromitovaných s limitací operačního výkonu. U pacientek s již diagnostikovanou malignitou by mohl napomoci v rozhodování v souvislosti s individualizací léčebného plánu a rozsahu chirurgického stagingu. Nevyřešená však zůstává otázka optimálního cut-off, k čemuž by měly přispět výsledky dalších rozsáhlejších studií a jejich komparativní analýza.

Abstract:

Background. Endometrial cancer is the most common gynaecological malignancy of Czech women. Despite the relatively favourable prognosis research focuses on the detection of markers for screening and early diagnosis, women at increased risk in particular could profit from it. The optimal surgical staging in the group of high risk patients is often limited by the age and internal co-morbidities that are usually combined with various grade of obesity. The markers that could help in distinguishing of patients with different prognosis and in individualizing the treatment procedure, are sought. Methods. Our study included 115 patients with endometrioid adenocarcinoma and 106 patients with benign endometrial lesions in the control group they were treated between June 2009 - July 2014. They were diagnosed through endometrial biopsy. All patients had serum HE4 and CA125 analysis after the biopsy. All patients with cancer underwent definitive surgical treatment. The median and ranges of serum levels were determined in relation to a histological result (benign vs malignant disease) and to a number of prognostic factors. Statistical analysis operates with logarithmic values of markers with respect to the distribution of HE4 and CA125 serum levels and uses logistic regression models. Results. While analysing two groups of patients with different histology, a statistically significant difference (p < 0.05) was demonstrated only in HE4; for cut-off 48.5 pmol/l, sensitivity of 87.8%, specificity of 56.6% and AUC of 0.773 was achieved. In terms of clinical practice, a statistically significant difference in HE4 (p < 0.05) has been detected in distinguishing of patients with low and high risk status of endometrial cancer. In our model achieving the maximum sum of sensitivity and specificity, HE4 shows a sensitivity of 72.4%, a specificity of 75.4% and AUC of 0.77 for the cut-off 76.5 pmol/l and is a better predictor in distinguishing the high-risk patients from ones than CA125. Conclusions. The diagnostic benefit of HE4 can be considered in combination with clinical and ultrasound finding especially in patients with increased risk of endometrial cancer and in patients with serious internal co-morbidities. In patients with cancer, HE4 can help in decision-making in relation to the individualization of the treatment plan and of the extent of surgical staging. However, the optimal cut-off for HE4 has not been solved yet, and to do so, it will require more research with larger studies and their comparative analysis.
