The issue of Family of Children with Orofacial Cleft

Abstract:

Cíl: Cílem práce je zjistit kvalitu života rodičů dětí s rozštěpovou vadou a vliv orofaciálního rozštěpu na rodinu. Dále porovnat zjištěnou kvalitu života rodičů s kvalitou života rodičů dětí bez rozštěpové vady ve věkovém rozmezí dětí 4-6 let. Metodika: Výzkumného šetření se zúčastnilo 172 respondentů. Výzkumný soubor tvořili rodiče dětí ve věku 4-6 let, které splňovaly daná kritéria. Z celkového počtu bylo 86 rodin dětí s orofaciálním rozštěpem a 86 rodin dětí bez orofaciálního rozštěpu. Dotazníkové šetření probíhalo se souhlasem náměstkyně pro ošetřovatelskou péči na ambulanci dětské plastiky a na ostatních ambulancích. Výzkum probíhal v období srpen-říjen 2018. K výzkumu byl použit standardizovaný dotazník PedsQL Family Impact Module verze 2.0, Vliv na rodinu. Autorem dotazníku je James W. Varni. Dotazník byl doplněn o demografické údaje. Analýza získaných dat byla prováděna pomocí dvouvýběrového t-testu na hladině statistické významnosti 0,05 (5%). Výsledky: Výzkum předložené diplomové práce byl zaměřen na problematiku rodiny dětí s orofaciálním rozštěpem. Zajímalo nás subjektivní vnímání těchto rodičů v psychosociální oblasti a v dopadu na funkci rodiny. Všechny hodnoty našeho výzkumu jsme porovnávali s daty, které jsme získali od rodiny dětí bez orofaciálního rozštěpu. Na základě těchto výsledků byl zjištěn statisticky významný rozdíl mezi rodiči dětí s orofaciálním rozštěpem a rodiči dětí bez orofaciálního rozštěpu v subjektivním vnímání fyzických, emocionálních, sociálních, poznávacích funkcí, v oblasti komunikace, obav a denních aktivit. Za největší problém rodiče dětí s orofaciálním rozštěpem považovali obavu o budoucnost svého dítěte. Na druhém místě rodiče dětí s orofaciálním rozštěpem uvedli, že se cítí unavení během dne. Nejméně dělal problém rodičům dětí s orofaciálním rozštěpem v oblasti fyzických funkcí bolest břicha. Pouze v oblasti rodinných vztahů jsme statisticky významný rozdíl nezjistili. Závěr: Na základě zjištěných výsledků vyplývá, že musíme uspokojovat nejen potřeby dítěte, ale i jeho rodičů. Je velmi důležité naslouchat rodičům dětí a umět jim poskytnou psychickou podporu. Nesmíme opomíjet komunikaci mezi rodiči a zdravotnickým personálem. Musíme si uvědomit, že péče o dítě s orofaciálním rozštěpem má dopad na celou rodinu.

Abstract:

Aim: The main aim of the final thesis is to detect subjective perception of the influence of the disease on the families of children with orofacial cleft regarding the health status of the child at 4 - 6 years of age and to compare it with families of children without orofacial cleft in the same age range. Method: The survey involved 172 respondents. The research group consisted of parents of children aged 4 - 6 who met the given criteria. There were 86 families with children with orofacial cleft and 86 families with children without orofacial cleft. The survey was executed with the agreement of the nursing care deputy in plastic surgery ambulance and other ambulances. The research was being accomplished from August to October 2018. A standardized questionnaire was used within the survey: PedsQL Family Impact Module version 2.0. The analysis was executed by two-tailed t-test with statistical significance 0,05 (5%). The questionnaire contains also demographical data. Results: The research of our final thesis was focused on problems in families of children with orofacial cleft. We focused on subjective perception of the parents in psychosomatic level and on the impact on their family function. The most serious problem is considered to be parents´ fears about the child´s future. The second most frequent response was tiredness during the day in parents with children with orofacial cleft. Stomach ache was mentioned as the least important problem. All the results of our research were compared to outcomes obtained from the families of children without the orofacial cleft. Having assessed the results in all domains we concluded that there is a statistically important difference between the two research groups of parents except for the item of "I have a stomach ache" where no statistically important difference was indicated. Conclusion: The results imply that we not only have to meet the child´s needs but also those of their parents. The importance of being able to listen to the children´s parents and to offer them psychical support is crucial. The communication between parents and medical staff must not be omitted. We have to realise that the orofacial cleft child care has an impact on the whole family.
