Evaluating the effectiveness of a linear accelerator from the perspective of a medical facility

Abstract:

Předkládaná diplomová práce se věnuje problematice hodnocení efektivity lineárního urychlovače z pohledu zdravotnického zařízení. Cílem práce bylo provést hodnocení nákladové efektivity pro účely budoucího nákupu lineárních urychlovačů ve zdravotnickém zařízení krajského typu. Za tímto účelem byl proveden rozbor nabídky lineárních urychlovačů v České republice včetně dekompozice legislativních požadavků. Pro detailní analýzu byly sledovány činnosti v rámci radioterapeutického procesu a vzniklé náklady i výnosy na oddělení radiační onkologie. Pro kalkulaci nákladů bylo využito smíšeného kalkulačního přístupu s využitím některých prvků metody Activity Based Costig. Významný vliv na efektivitu lineárních urychlovačů mají vysoké pořizovací a provozní náklady, proto byly při zpracování použity dynamické metody hodnocení efektivnosti investic. Z perspektivy zdravotnického zařízení se investice do lineárních urychlovačů dle metodického doporučení jeví jako vysoce výnosné s krátkou dobou návratnosti vynaložené investice. Výsledky diplomové práce ukazují, že pomocí technologicky pokročilejších technik radioterapie a alternativních frakcionačních schémat, lze regulovat čas potřebný k výkonům v rámci radioterapeutického procesu a tím lze dosáhnout dalšího zvýšení efektivity

Abstract:

The present thesis deals with the evaluation of the efficiency of a linear accelerator from the point of view of a medical facility. The aim of the thesis was to carry out a cost-effectiveness evaluation for the purpose of future purchases of linear accelerators in a regional healthcare facility. For this purpose, an analysis of the supply of linear accelerators in the Czech Republic was carried out, including the decomposition of legislative requirements. For a detailed analysis, the activities within the radiotherapy process and the costs and revenues incurred in the radiation oncology department were monitored. A mixed costing approach using some elements of the Activity Based Costing method was used for costing. The high acquisition and operating costs have a significant impact on the efficiency of linear accelerators, therefore dynamic methods were used to assess the efficiency of the investment. From the perspective of a healthcare facility, investment in linear accelerators according to the methodological recommendation appears to be highly profitable with a short payback period. The results of the thesis show that by using technologically more advanced radiotherapy techniques and alternative fractionation schemes, the time required for the radiotherapy process can be regulated and thus further efficiency improvements can be achieved.
