Analysis of myeloma cell progenitors

Abstract:

Mnohočetný myelom (MM) je zhoubné nádorové onemocnění krve, jehož podstatou je maligní transformace B-lymfocytu (B-ly), jeho nekontrolovaná proliferace a diferenciace do patologických plazmocytů (PC), které se akumulují v kostní dřeni. Tomuto malignímu onemocnění téměř ve všech případech předchází benigní prekanceróza monoklonální gamapatie nejasného významu (MGUS). Přes veškeré pokroky ve vývoji nových léčebných postupů je MM stále velmi obtížně léčitelné onemocnění. Jednou z možných příčin relapsu onemocnění po dosažení remise je existence tzv. myelom-iniciujících buněk (MIC), vykazujících rezistenci na léčbu, schopnost sebeobnovy a potenciál pro vznik nádoru, nicméně přesný fenotyp těchto buněk nebyl doposud spolehlivě identifikován. Je zřejmé, že pro objasnění patofyziologie MM, stejně jako pro identifikaci MIC je nezbytná podrobná analýza subpopulací B-ly, jakožto prekurzorových stádií maligních PC. Disertační práce byla zaměřena na detailní analýzu subpopulací B-ly u pacientů s monoklonálními gamapatiemi stran fenotypu, zastoupení a klonality s využitím multiparametrické průtokové cytometrie. V další části práce byly na jednotlivých subpopulacích B-ly analyzovány vlastnosti typické pro kmenové buňky jako je aktivita enzymu aldehyd-dehydrogenázy (ALDH) a vybrané subpopulace byly izolovány a podrobeny analýze stanovení příslušnosti k malignímu klonu PC pomocí detekce specifické přestavby těžkého imunoglobulinového řetězce (IgH). V rámci analýz byl podrobně popsán fenotyp a zastoupení subpopulací B-ly a PC jak u pacientů s MM, tak u osob s MGUS. Srovnání zastoupení subpopulací mezi pacienty v různém stádiu onemocnění a kontrolních jedinců prokázalo klesající trend procentuálního zastoupení B-ly od benigního MGUS po nejpokročilejší stádia MM. Tento parametr by mohl do budoucna sloužit jako ukazatel progrese onemocnění Analýzy klonality jednotlivých subpopulací odhalily značnou variabilitu v rámci diagnózy MM a MGUS a to zejména odlišnost IgM monoklonálních gamapatií (MG) od non-IgM MG. Stanovení klonality a fenotypového profilu pak vedlo ke zpřesnění diferenciální diagnostiky jak monoklonálních gamapatií, tak i dalších lymfoproliferativních onemocnění. Stanovení klonální příslušnosti B-ly pomocí detekce specifické přestavby IgH prokázalo klonotypické B-ly u jedné poloviny pacientů, nicméně fenotypová variabilita těchto buněk neumožňovala jednoznačnou identifikaci zdrojové populace potenciálních MIC. V průběhu diferenciace B-ly byly u pacientů s MM i MGUS pozorovány změny v aktivitě enzymu ALDH, nicméně stejné změny byly zaznamenány také u zdravých jedinců. Zvýšená aktivita ALDH tedy není vhodným samostatným markerem pro identifikaci MIC. Získané výsledky přinesly zcela nové informace o fenotypu, zastoupení i dalších vlastnostech jednotlivých subpopulací, které nebyly v průběhu vývoje B-ly u pacientů s monoklonálními gamapatiemi doposud detailně popsány. Analýzy také vedly k implementaci protokolů pro diagnostiku některých lymfoproliferativních onemocnění do rutinní laboratorní praxe.

Abstract:

Multiple myelom (MM) is a hemato-oncological malignancy characterized by malignant transformation of B-cell, its proliferation and differentiation into plasma cells (PC) accumulating in the bone marrow. In almost all cases, MM is preceded by benign monoclonal gammopathy of undetermined significance (MGUS). Despite all advances in development of new therapeutic approaches, MM is still very difficult to treat. Frequent relapses are thought to be associated with existence of the so-called myeloma initiating cells (MIC) that exhibit resistence to therapy, self-renewal and tumour-initiating potential. However, the exact phenotype of these cells has not been reliably identified so far. It is evident that detailed analysis of B cell subpopulations, as precursors of malignant PC, is required to clarify pathophysiology of MM as well as to identify MIC. This dissertation was focused on a detailed analysis of B-cell subpopulations in patients with monoclonal gammopathies in terms of phenotype, representation and clonality using multiparametric flow cytometry. In the second part of the thesis, typical stem cell characteristics, such as activity of aldehyde dehydrogenase (ALDH), have been analyzed in individual subpopulations; selected subpopulations were isolated and subjected to analysis of the affiliation to the malignant clone PC using detection of specific immunoglobulin heavy chain (IgH) rearrangement. Within our analysis, phenotype and representation of B-cell and PC subsets was described in MM as well as MGUS patients. Comparison of B-cell subsets representation among patients in different disease stages and healthy controls shows a decreasing trend in percentage of B-cells from benign MGUS to the most advanced stages of MM. This parameter would eventually be an indicator of disease progression. Clonality analysis of individual subpopulations revealed significant variability in MM and MGUS cases, in particular difference of IgM monoclonal gammopathies (MGs) from non-IgM MGs. Clonality and phenotype profile assessment lead to more accurate differential diagnosis of both monoclonal gammopathies and other lymphoproliferative disorders. Determination of clonal affiliation of B-cells using specific IgH rearrangement detection showed clonotypic B-cells in half of the patients; however, phenotypic variability of these cells did not allow clear identification of potential MIC source populations. Changes of ALDH activity were observed during B-cell development in MM as well as in MGUS patients; however, the same changes were detected also in healthy controls. Therefore, high ALDH activity as a single marker is not appropriate for MICs identification. The obtained results provide completely new information about phenotype, representations as well as other characteristics of individual B-cell developmental stages that were not previously described in detail in monoclonal gammopathies. Analysis also led to the implementation of protocols for the diagnosis of certain lymphoproliferative disease in routine laboratory practice.
