Improvements in diagnosis and treatment of sever opportunistic fungal infections in patients with haematological malignancy and after stem cell transplantation

Abstract:

Jednou z nejčastějších komplikací nemocných s hematologickou malignitou jsou infekce. Mezi nimi nabývají stále většího významu infekce mykotické – zejména invazivní aspergilóza, která je dnes hlavní invazivní mykózou u nemocných po alogenní transplantaci krvetvorné tkáně a pacientů léčených pro akutní leukémii. Pro prognózu nemocných s invazivní mykózou má zásadní význam včasná diagnóza a včasné zahájení účinné antimykotické léčby. Předkládaná práce je zaměřena na: 1. Analýzu využitelnosti detekce galaktomananu pro časnou diagnostiku invazivní aspergilózy u 911 nemocných, která prokázala vynikající senzitivitu, specificitu a negativní prediktivní hodnotu testu, ale omezenou pozitivní prediktivní hodnotu způsobenou především četnými falešnými pozitivitami. Blíže rozebírány jsou příčiny falešných pozitivit a možnosti jejich eliminace. 2. Analýzu využitelnosti detekce “panfungálního” markeru 1,3-ß-D-glukanu v časné detekci hlubokých mykóz během 104 léčebných cyklů, která prokázala omezenou použitelnost metody v rutinní diagnostice mykóz. Test je možné využít pro konfirmaci pozitivity galaktomananu při diagnostice invazivní aspergilózy 3. Podrobný rozbor 52 případů invazivní aspergilózy diagnostikovaných v letech 2000-2006 na našem pracovišti, který prokázal klíčovou roli rutinního monitoringu galaktomananu a velkou efektivitu kombinované antimykotické léčby.

Abstract:

Infections are one of the leading complications of patients with hematological malignancy. Invasive fungal infections – especially invasive aspergillosis - have become one of the major causes of morbidity and mortality mostly in patients with acute leukemia and after stem cell transplantation. Early diagnosis and prompt initiation of antifungal treatment are leading factors influencing prognosis of patients with invasive fungal infection. Our work was focused on: 1. Evaluation of usefulness of galactomannan detection in serum for early diagnosis of invasive aspergillosis in 911 patients showed high sensitivity, specificity and negative predictive value, but on the other hand low positive predictive value, caused mainly by frequent false positive results. Sources of this false positivity and possibilities of their elimination are discussed. 2. Evaluation of usefulness of 1,3-ß-D glucan detection for screening of invasive fungal infections in 104 treatment cycles that clearly proved limited potential of the test in daily use. On the other hand, 1,3-ß-D glucan testing seems to be promising tool for confirmation of consecutive galactomannan positive result in serum in patients with invasive aspergillosis. 3. Detailed analysis of 52 cases of invasive aspergillosis in our department between 2000-2006 that showed key role of galactomannan detection in early diagnosis of this infection and important efficacy of combine antifungal treatment in this setting.
