Abstract:

Infekce představuje závažnou komplikaci po cévních rekonstrukčních výkonech. V předkládané práci jsou zhodnoceny současné výsledky péče o pacienty, léčené na II. chirurgické klinice Fakultní nemocnice u svaté Anny v Brně. Předkládáme souhrn provedených diagnostických a léčebných opatření u pacientů, operovaných na naší klinice v průběhu let 2003-2006 pro nově zjištěnou infekci tepenné rekonstrukce. Další sledování souboru pacientů trvalo do konce roku 2009. V našem souboru šlo o 22 mužů a 5 žen, průměrný věk v době manifestace infekce byl 62,2 roku. Vždy šlo o infekci III. stupně dle Szilagyiho klasifikace. Ve stejném období bylo provedeno 746 primárních tepenných rekonstrukcí v oblasti aorty a tepen dolních končetin. Incidence infekce představuje tedy 1,21% cévních operací. K projevům infekčních komplikací došlo v rozpětí od několika dnů až po 15 let od poslední rekonstrukční cévní operace. Infekce se nejčastěji objevila jako retence purulentního sekretu v třísle (63%). Původcem byl nejčastěji Staphylococcus aureus (48,1%). V 18,5% případů nebylo bakteriální agens zjištěno. Léčba spočívala nejčastěji (51,9% případů) nejdříve v drenáži infikované lokality. Následně byla provedena explantace infikované rekonstrukce s event. náhradou extra-anatomicky vedeným bypassem. Časné zachování končetiny představuje 74,1%, časná mortalita 11,1%. Sledování probíhalo do konce roku 2009, nejméně 36 měsíců. Úmrtnost do 1 roku představuje 22,2%. V současnosti žije 19 pacientů z tohoto souboru. Naše výsledky jsou plně srovnatelné s jinými pracovišti cévní chirurgie. Přesto existují rezervy v časné diagnostice i léčebných opatřeních, na které v práci upozorňujeme.

Abstract:

Infections represent a serious complication in the reconstructive vascular surgery. In this thesis we evaluate results obtained as part of treatment of patients of the 2nd Department of Surgery, St. Anne's University Hospital Brno. We summarize the diagnostic and treatment procedures in patients who were operated at the 2nd Department of Surgery between years 2003 and 2006 for a recent development of infection of arterial reconstruction, with a further follow-up until the end of 2009. There were 22 men and 5 women of an average age of 62,2 years when the infection developed. All the cases represented the 3rd grade of the Szilagyi’s classification. In the same period 746 primary arterial reconstructions in the region of abdominal aorta and lower limb arteries were performed. The incidence of the infection reached 1,21% of all our vascular operations. The manifestation of the infection occurred from a few days to 15 years after the last vascular reconstruction. The infection manifested mainly as a purulent retention in the groin (63%) and was caused mainly by Staphylococcus aureus (48,1%). No bacterial agent was detected in 18,5%. The treatment was most frequently (51,9%) by way of draining the infected area first, followed by a removal of the reconstruction, with or without a replacement using extra-anatomic reconstruction. The early limb preservation reached 74,1% and early mortality 11,1%. The patients were followed up until the end of 2009 i.e. at least 36 months. The one-year mortality rate reached 22,2%. Today 19 patients are still alive. The results obtained are fully comparable to those from other vascular surgery centres. However, there still exists a considerable space for improvement of the early diagnostics and treatment, which have both been describe and commented on in the thesis.
