The effect of application of botulinum toxin to the affected muscles on the stereognosis in patients with cervical dystonia

Abstract:

Diplomová práce se zabývá vlivem botulotoxinové léčby na stereognostickou funkci a kinestezii u pacientů s cervikální dystonií a zhodnocením relevantnosti použití vybraného testování pomocí přístroje DeskBalance a modifikovaného testu dle Petrie k objektivizaci efektu terapie v rámci běžné praxe. Soubor se skládal z 6 žen (56,00 ± 6,39) a 2 mužů (50,50 ± 14,85) ambulantně léčených pro diagnózu cervikální dystonie na Klinice tělovýchovného lékařství a rehabilitace u sv. Anny v Brně. Výsledky naměřené pomocí přístroje DeskBalance a modifikovaného testu dle Petrie před aplikací botulotoxinu do postiženého svalstva byly srovnávány vůči těm dosaženým po aplikaci botulotoxinu s odstupem 6–8 týdnů v době největšího ovlivnění, a to dohromady pro obě horní končetiny i pro každou končetinu zvlášť, diferencovanou na základě zvoleného kritéria. Statisticky významné rozdíly mezi sledovanými parametry byly prokázány pouze v případě, kdy se hodnotila změna stereognozie a kinestezie pomocí přístroje DeskBalance před a po aplikaci botulotoxinu pro obě horní končetiny a zvlášť pro končetinu na straně, kam se probandům stáčela hlava. I přes to, se z praktického pohledu, časové a technické nenáročnosti přikláníme spíše k možnosti posouzení vlivu léčby cervikální dystonie v rámci běžné praxe pomocí modifikovaného testu dle Petrie.

Abstract:

This master's thesis deals with the effect of botulinum toxin treatment on stereognostic function and kinesthesia in patients with cervical dystonia and evaluating the relevance of using selected testing using DeskBalance and modified Petrie test to objectify the effect of therapy in common practice. The group consisted of 6 women (56,00 ± 6,39) and 2 men (50,50 ± 14,85) treated as outpatients for the diagnosis of cervical dystonia at the Clinic of Sports Medicine and Rehabilitation in St. Anna in Brno. The results measured with the DeskBalance and the modified Petrie test before botulinum toxin application to the affected muscles were compared to those obtained after botulinum toxin administration at 6-8 weeks apart at the time of the greatest effect, together for both upper limbs and for each limb separately, differentiated into based on the selected criterion. Statistically significant differences between the endpoints were shown only when the change in stereognosis and kinesthesia was evaluated with the DeskBalance before and after botulinum toxin administration for both the upper limbs and especially the limb on the side where the probands’ heads were twisted. Due to the time and technical demands, we tend to assess the impact of cervical dystonia treatment in the common practice using a modified Petrie test.
