Nurses'knowledge about the problems of delirium

Abstract:

Znalosti sester v problematice delirantních stavů je velice důležité téma. Několikrát jsem se v praxi sama setkala s tím, jak je složité rozpoznat hypoaktivní delirium u pacienta upoutaného na lůžko. Díky anonymním dotazníkům v rámci dotazníkového šetření ve dvou zdravotnických zařízeních se sestrám tato problematika opět přiblíží a domnívám se, že tím v některých případech napomohu ke správnému diagnostikování deliria. Je důležité, aby sestra včas odhalila příznaky, tím pomohla zabránit jejich rozvinutí a přispěla ke zkrácení doby hospitalizace. V teoretické části práce se zabývám deliriem obecně a úlohami sester u pacienta, kvalitní komunikací a přístupem k pacientovi, rovněž dělením léčby na nefarmakologickou a farmakologickou podle přístupu zdravotnického týmu. V rámci praktické části se věnuji dotazníkovému šetření. Hlavním cílem bylo zjistit, zda jsou sestry s tímto tématem dostatečně seznámeny. Průzkumné šetření probíhalo v Nemocnici následné péče Ryjice v Ústí nad Labem - Neštěmice a v Masarykově nemocnici v Ústí nad Labem. Výsledky byly zpracovány do tabulek a grafů. Během ledna a února roku 2015 bylo osloveno celkem 90 respondentů z řad zdravotních sester obou zdravotnických zařízení. Využitelnost dotazníků byla 100%. Na základě výsledků byl zpracován edukační poster pro sestry, který by měl být vyvěšen na odděleních nemocnice, aby napomáhal sestrám ve zlepšení okolí pro pacienta a tím i prevencí deliria. V rámci celkového hodnocení dotazníkové šetření jsem zjistila, že nejlepší výsledky v oblasti znalosti delirantních stavů mají sestry z geriatrických oddělení, následovány sestrami z jednotek intenzivní péče.

Abstract:

Nurse´s knowledge of delirium state is a very important theme. Few times I could see comlexit yof discerning a hypoactive delirium state on lying pacient. This is the mason why I chose this topic because even through anonymous question naires, which I distributed to the wards, the nurses approaching this is sue again and I think this will help to properly diagnose delirium and reduced hospitalization for patients. It is important that the nurse can early detect symptoms and this help to short the time of hospitalization. The theoretical part deals with the general delirium and nurses' role in the patient, proper communication and access to the patient. Another point treatment is dividend into non-pharmacological and pharmacological approach by the medical team. In the practical part is devoted to a question naire survey. The main objektive was to determine whet her the nurses are unfamiliar with this topic. The surfy was conducted in NNP Ryjice and department soft he Masaryk Hospital in Ústí nad Labem and there sults were summarized in tables and graphs. I spoke a total of 90 respondents during January and February. The overall response was 100%. Based on there sults was prepared educational poster for nurses, which should be posted on the department of the hospital and assist in improving and sister arend the patient and thus preventiv of delirium. Results of the evaluation of the overall and after wards, where the best results have geriatric ward and after them the intensive care unit.
