The therapeutic exercise for the cardiological rehabilitation of a patient with acute myocardial infarction: clinical case

Abstract:

The cardiac patients affected by acute myocardial infarction hardly modifies his lifestyle, in an important way, after being dismissed by the hospital, even though sometimes an attempt like this is set in the following months. The clinical case taken over concerns an old patient of 78 years, affected by an important ischemic cardiomyopathy after a STEMI coronary artery disease in January 2016 and a NSTEMI coronary artery disease in December 2016. After this the woman endured 4 angioplasties and several recoveries due to anginal symptoms similar to the ones above, but fortunately, without the same evolution. In this paper, after having explored the theoretical aspects of the pathology, we dwelled on the physiotherapic intervention after 3 years since the last important coronary artery intervention. As described in his professional skills, the physiotherapist is charged with the rehabilitation of the patient, in order to conduct a normal life. Moreover he should take care of the prevention of new possible episodes and help the patient with the improvement of his own living standards. Starting from this, we analysed in depth the current risk factors: we gave our patient some scientifically validated ratings in order to classify the quality of her own life and then we gave her some tests for the ratings of her psycho-physic health' levels. Furthermore, with the patient, we developed a 4-weeks programme, aimed at having a vision of continuity after this period. The programme included 3 blocks of work: the first one at home independently, the second one at the physiotherapist studio and the third one a is a block of treatment which uses a technological innovation, the Bioresonance, aimed at reducing one of the most evident risk factor, such as stress. The confrontation of both the results of tests done at the end of the process and the ones obtained before with the patient's interview, offer significant indications on the importance of what we proposed. The valuation of the same parameters, keeping only the domestic work, is referred to a further study.

Il paziente cardiologico affetto da infarto miocardico acuto difficilmente modifica in maniera importante il proprio stile di vita dopo essere stato dimesso dall'ospedale, anche se a volte un tentativo in questo senso viene impostato nei primi mesi seguenti. Il caso clinico preso in carico riguarda una paziente anziana di 78 anni affetta da un'importante cardiomiopatia ischemica conseguente ad una coronaropatia STEMI nel gennaio 2016 ed una coronaropatia NSTEMI nel dicembre 2016. Successivamente, la signora ha subito 4 angioplastiche e diversi ricoveri dovuti a sintomatologie anginose simili a quelle presenti durante gli eventi soprariportati, ma, fortunatamente, senza la medesima evoluzione. In questo elaborato, dopo aver approfondito gli aspetti teorici riguardanti la patologia in essere, ci siamo soffermati sull'intervento fisioterapico dopo 3 anni dall'ultimo evento coronaropatico importante. Il fisioterapista, come descritto nelle proprie competenze professionali, oltre ad avere il compito di riabilitare il paziente a svolgere una vita definita normale, deve occuparsi della prevenzione di nuovi episodi, nonché aiutare aiutarlo ad ottimizzare la propria qualit? di vita. Partendo da questo principio abbiamo analizzato in maniera approfondita i fattori di rischio presenti, abbiamo somministrato alla nostra paziente delle valutazioni scientificamente validate per classificare la qualit? della propria vita ed abbiamo somministrato dei test per la valutazione del livello di salute psico-fisica. Successivamente, assieme alla paziente, ? stato elaborato un programma della durata di 4 settimane con l'obiettivo di avere una visione di continuit? conseguente a questo periodo. Il programma comprendeva tre blocchi di lavoro: uno domiciliare in autonomia, uno presso lo studio fisioterapico ed un blocco di trattamento utilizzando una innovativa tecnologia, la biorisonanza, con l'obiettivo di ridurre uno dei fattori di rischio pi? evidenti in questo caso, lo stress. Il confronto dei risultati dei test effettuati a fine percorso con quelli ottenuti prima assieme al colloquio con la paziente offrono evidenti indicazioni sull'importanza di quanto proposto. Si rimanda ad un successivo studio la valutazione dei medesimi parametri mantenendo il solo lavoro domiciliare.
