Forensic view on the SIDS in the Czech Republic

Abstract:

Dizertační práce předkládá systematický a aktuální přehled o problematice Sudden Infant Death Syndrome (SIDS). Uvádí současný pohled zejména zahraničních odborníků na náhlá úmrtí kojenců, prezentuje v současnosti platnou definici, známé rizikové faktory, teorie o možných příčinách těchto úmrtí, preventivní a intervenční opatření, stejně tak obvyklý patologický nález. Součástí práce je soubor náhlých úmrtí kojenců v rámci celé České republiky, během let 1999 – 2004. Tento soubor byl získán zpracováním zdrojových dat, z Listů o prohlídce mrtvého, získaných z Ústavu zdravotnické informatiky a statistiky při Ministerstvu zdravotnictví ČR. V rámci celé České republiky taková data nebyla doposud zhodnocena a zpracována. Sledovány byly jednotlivé epidemiologické rizikové faktory, jako pohlaví, věk, sezónnost, výskyt v jednotlivých krajích ČR, byla vypočítávána incidence SIDS. Zjištěné údaje byly porovnávány s závěry odborné literatury a byly dále konfrontovány s údaji, vycházejícími z pitevních protokolů ÚSL LF MU a FNUSA v Brně. Práce obsahuje mezinárodní doporučení, tzv. „guidelines“ při úmrtí kojence.

Abstract:

The PhD thesis provides systematic and actual overview on the problem of Sudeen Infant Death Syndrome (SIDS). Opinions of the leading foreign experts in sudden infant deaths, including most recent SIDS definition, known risk factors and theories explaining possible causes of death in SIDS are presented. Preventive and intervention measures are summarized as well as the usual pathological findings in SIDS. Substantial part of the thesis analyses cases of infant deaths on the territory of the Czech Republic in the years 1999 - 2004. No previous analysis of these deaths exists has been ever performed in the Czech Republic. The source data were obtained from the death certificates („List o prohlídce zemřelého“) provided by the Institute of Health Information and Statistics of the Czech Republic („UZIS data“). Data were confronted with the available literature, processed and later compared with the autopsy protocols of the Institute of Forensic Medicine of the Masaryk University's Faculty of Medicine and St. Anne's University Hospital in Brno (USL). Various epidemiological factors as age, gender, seasonality, regional distribution have been evaluated and incidence has been calculated. The presented thesis includes guidelines for forensic patholgists how to investigate cases of death in infants.
