Abstract:

Za nerovnosti ve zdraví jsou označovány systematické rozdíly mezi sociálními skupinami, jež se liší svou socioekonomickou pozicí, místem, kde žijí, genderem či etnickou příslušností. Tyto nerovnosti poutají zájem zejména proto, že většinou odrážejí nerovnou distribuci zdrojů ve společnosti. Rozdíly ve zdraví, které se odvíjí od nerovného postavení sociálních skupin, jsou vnímány jako důsledek nerovných šancí na zdraví a jako takové jsou většinou považovány za nespravedlivé. Největší pozornost poutá silná souvislost mezi socioekonomickou pozicí a zdravím. Existují čtyři základní vysvětlení tohoto vztahu – teorie artefaktu, teorie výběru, behaviorální vysvětlení a vysvětlení strukturální. V současnosti narůstá konsensus, že nerovnosti ve zdraví jsou důsledkem uspořádání společnosti. Nejvíce prostoru je proto věnováno strukturálním vysvětlením vycházejícím z konceptu sociálních determinant zdraví, především jejich psychosociální verzi. Materiální podmínky byly nejdůležitější determinantou zdraví v době dominance infekčních nemocí. Růst životní úrovně a rozvoj medicíny vedl k epidemiologické tranzici a s ní zřejmě i k proměně hlavních sociálních determinant zdraví. Existence sociálního gradientu ve zdraví navíc napovídá, že rozdíly ve zdraví nejsou pouze otázkou rozdílu mezi chudými a zbytkem společnosti. Sociální gradient ve zdraví je vysvětlován pomocí konceptu relativní deprivace, zejména pak deprivace sociální. V souladu s ním jsou sociální opora, pocit kontroly nad vlastním životem, povaha práce či sociální koheze považovány za nejvýznamnější sociální determinanty zdraví. Zdá se, že dobré interpersonální vztahy, smysluplnost života a participace na životě společnosti jsou klíčové pro zdraví lidí ve vyspělých společnostech. Ačkoli je největší pozornost věnována socioekonomickým nerovnostem ve zdraví, je třeba se zabývat rovněž připsaným charakteristikám jako je gender nebo etnická příslušnost jako významným strukturálním determinantám zdraví.

Abstract:

The dissertation deals with social determinants of inequalities in health. The term inequalities in health refers to systematic differences in health between social groups defined by socioeconomic position, place, gender, ethnicity etc. Most of the inequalities in health across social groups are unjust because they reflect an unfair distribution of the underlying social determinants of health. There are for main types of explanations of strong relationship between socioeconomic position and health – artefact hypothesis, selection, behavioral explanation and structural explanation. Current research suggest that material, cultural and psychosocial determinants are critical links between social structure and health. The unequal social distribution of these determinants represents the specific responses of socioeconomic groups to the conditions imposed on them by social structure. Material conditions were the most important determinant of health at times of infection diseases domination. Growth of life standard and medicine development led to epidemiological transition and with it as the case may be even to a change of the main social determinants of health. Existence of social gradient in health suggests that differences in health are not only question of difference between the poor and the rest of society. Social gradient can be explained by the concept of relative deprivation, especially social deprivation. Social support, perceived control over one’s own life, psychosocial environment at work, full social participation or social cohesion, all this required to be the most important social determinants of health. These statements have significant implications for health policy that must seek out specific strategies tackling social determinants of inequalities in health.
