Abstract:

Rezistence na antibiotika je poslední dobou velmi skloňovaný problém, který začíná být stále závažnější. Antibiotika se právem řadí k nejvýznamnějším medicínským objevům 20. století a jejich použití vedlo k vymizení řady chorob, či alespoň k výraznému snížení jejich výskytu a nebezpečnosti. Schopnost bakterií bránit se účinku antibiotik je z tohoto důvodu obávaným problémem. V posledních letech ustupují do pozadí klasické rezistence jako methicilin rezistentní Staphylococcus aureus, nebo širokospektré laktamasy. Předmětem zájmu se naopak stávají rezistence k záložním antibiotikům, jako jsou karbapenemy a kolistin. Tato práce přehledně zpracovává problematiku antibiotik, jejich klasifikaci a použití. Dále se zabývá mechanismy rezistencí, jejich přenosem a popisuje nejvýznamnější rezistence posledních let. Praktická část je zaměřena na detekci produkce karbapenemas u enterobakterií na základě stanovení fenotypu za použití inhibitorů.

Abstract:

Resistance to antibiotics has been a very discussed problem, which is becoming more serious in last years. Antibiotics are rightly ranked among the most important medical discoveries of the 20th century, and their use has led to the disappearance of many diseases, or at least to a significant reduction in their incidence and dangerousness. Therefore, the ability of bacteria to prevent antibiotics is a dreaded problem. In last years classical resistance, such as methicillin-resistant Staphylococcus aureus or broad-spectrum lactamases, has moved backward. On the contrary, the subject of interest becomes resistance to "last line" antibiotics such as carbapenems and colistin. This thesis clearly deals problematic with antibiotics, their classification, and use. It also deals with resistance mechanisms, their transmission and describes the most significant resistance of recent years. The practical part is focused on the detection of the production of carbapenemases in enterobacteria based on the determination of the phenotype using inhibitors.
