Fast Track Technique in Intestinal Surgery

Abstract:

Cíl: Cílem diplomové práce je zjistit, zdali metoda Fast Track snižuje pooperační komplikace, urychluje hospitalizaci a zvyšuje komfort pacientů po střevních operacích. Metodika: Cílovou skupinu tvořilo 100 pacientů (50 Fast Tracku a 50 v klasické přípravě) ve věku od 18 do 87 let věku, u kterých byl od 1. 7. - 30. 10. 2019 proveden operační výkon na střevech, a to ve dvou vybraných nemocnicích Moravskoslezského kraje. Sběr dat byl realizován pomocí měřícího nástroje Fast Track. Výsledky: Průměrný věk tak činil u mužů 58,3 let a u žen 52,67 let. Nejčastější diagnózou při příjmu byla karcinom rekta (C 20) u Fast Tracku až v 50 % a u klasické přípravy to bylo 40 %. Peristaltika byla obnovena ve Fast Tracku u 56 % do 13-24 hodin od operace a u klasické metody to bylo do 25-36 hodin (46 %) po operaci. Rychlejší obnova soběstačnosti byla u metody Fast Track a to 2-3 den po operaci. Pooperační komplikace mělo 22 % u Fast Tracku a 36 % pacientů u klasické přípravy. Nutnost podávání analgetik pooperačně byla u Fast Tracku nejčastěji v rozmezí 6-24 hodin a u klasické metody 1-3 pooperační den. Průměrná délka hospitalizace byla 8,14 u Fast Tracku a 10,12 dní u klasické metody. Závěr: Šetřením jsme zjistili, že metoda Fast Track je jak pro seniory, tak i pro mladší jedince výhodnější v mnoha ohledech než příprava klasická.

Abstract:

Introduction: The aim of the diploma thesis is to find out, whether the Fast Track (also known as ERAS) reduces postoperative complications, shortens hospital stay and improves patient comfort after intestinal surgery. Methodology: For the research it has been chosen 100 respondents (50 Fast Track and 50 in classic preparation) after intestinal surgery from 18 to 80 years. The research took place from 1. July to 30. October 2019, and involved two hospitals in Moravskoslezský kraj. Data collection was performed by using Fast Track measurement tool. Results: The average age of the respondents was 58, 3 years for men and 52, 67 years for women. Colorectal cancer (C 20) was the most frequent diagnosis, 50 % in the Fast track group and 40 % in the conventional care group. In the Fast Track group, the intestinal peristalsis occurred within 13 - 24 hours after surgery (51%) and in the conventional care group, the return of peristalsis was observed within 25 - 36 hours after surgery (46%). On the other hand, in the Fast Track group, early mobilization and self-sufficiency within 2 - 3 days has been observed. In the Fast Track group, 22% of patients had postoperative complications whereas in the conventional care group, 36 % of patients suffered postoperative complications. The average length of pharmacological analgesia administration was much shorter in the Fast Track group compared with conventional care group (6 - 24 hours vs 1 - 3 days). The average hospital stay was 8, 14 days in the Fast Track group and 10, 12 days in the conventional care group. Conclusion: Compared to the traditional management, the Fast Track method is much more beneficial in many respects for both seniors and younger individuals. Patients undergoing traditional preoperative preparation obviously incline to slower mobilization and oral intake, increased incidence of PONV, slower recovery of bowel movement, higher incidence of postoperative complications as well as greater pain intensity and longer hospital stay. Also, the incidence of abdominal drainage and permanent urine catheters is higher in traditional management than in Fast Track method.
