Protilátkový JCV index u pacientů s RS léčených imunomodulační léčbou - prevalence, dynamika a význam

Antibody JCV-index in patients with multiple sclerosis treated with disease-modifying therapy

Abstract:

Roztroušená skleróza je chronické demyelinizační onemocnění v jejíž patogenezi se předpokládá zásadní podíl autoimunitních dějů, při kterých dochází k poškození bílé i šedé hmoty centrálního nervového systému. Natalizumab, humanizovaná monoklonální protilátka, je inhibitorem a4b1 integrinu, která se používá pro léčbu vysoce aktivní relaps-remitentní roztroušené sklerózy. Bohužel tato velmi účinná léčba je spojena s rizikem progresivní multifokální leukoencefalopatie, závažné virové oportunní infekce způsobené John Cunningham virem (JCV). V rámci stratifikace rizika této oportunní infekce se u pacientů léčených natalizumabem hodnotí protilátkový JCV index. Cílem uvedeného souboru komentovaných prací bylo zejména přispět k lepšímu náhledu na komplexní problematiku progresivní multifokální leukoencefalopatie u pacientů s roztroušenou sklerózou léčených imunomodulační léčbou. Studie shrnuje informace o tomto závažném virovém onemocnění a diskutuje význam protilátkového JCV indexu a rozšiřuje informace o tomto biomarkeru stran prevalence pozitivity a sérokonverze v naší kohortě pacientů a navrhuje natalizumabem indukovanou lymfocytózu jako další potentionální biomarker ke stratifikaci rizika progresivní multifokální leukoencefalopatie.

Abstract:

Multiple sclerosis is a chronic demyelinating disease, the pathogenesis of which is thought to involve a significant proportion of autoimmune processes in which the white and gray matter of the central nervous system is damaged. Natalizumab, a humanized monoclonal antibody, is an inhibitor of the α4b1 integrin used to treat highly active relapsing-remitting multiple sclerosis. Unfortunately, this very effective treatment is associated with the risk of progressive multifocal leukoencephalopathy, a serious viral opportunistic infection caused by the John Cunningham virus (JCV). As part of the risk stratification, the antibody JCV index is assessed in patients treated with natalizumab. The aim of this set of commented studies was to contribute to a better insight into the complexity of progressive multifocal leukoencephalopathy in patients with multiple sclerosis treated with disease modifying therapies. The study summarizes information about this viral disease and discusses the significance of the antibody JCV index and expands information on this biomarker in terms of positivity prevalence and seroconversion in our cohort of patients and suggests natalizumab-induced lymphocytosis as another possible biomarker to stratify the risk of progressive multifocal leukoencephalopathy.
