Sledování nutričního a psychického stavu u nemocných s Alzheimerovou chorobou - vliv nutriční intervence v longitudinální studii

Influence of nutritional support on nutritional and mental status in dementia patients with Alzheimer's disease: comparative and intervention study

Abstract:

 U pacientů s Alzheimerovou chorobou je stav výživy horší než u nemocných s demencí vaskulární etiologie, a to přestože příjem potravy je u obou skupin nemocných srovnatelný.  Hodnocení stavu výživy pomocí MNA (mini nutritional assessment – metoda hodnocení stavu výživy vyvinutá pro geriatrickou populaci) je možno použít i u nemocných s Alzheimerovou chorobou. O tom svědčí korelace mezi antropometrickými parametry a hodnotami MNA zjištěnými u našeho souboru nemocných.  Hodnoty MNA korelují se skutečným příjmem potravy - jak u nemocných s vaskulárním postižením, tak u pacientů s Alzheimerovou chorobou. Z uvedeného nálezu vyplývá, že vyšetření rizika vzniku malnutrice pomocí MNA je možno použít i u nemocných s Alzheimerovou chorobou.  Sérové hladiny cholesterolu u nemocných s Alzheimerovou chorobou se nelišily od hodnot pacientů s demencí vaskulární etiologie. Hladina cholesterolu však korelovala s hodnotou MNA, což svědčí o tom, že malnutrice doprovázející Alzheimerovu chorobu může být příčinou snížené hladiny cholesterolu u těchto pacientů.  Z necholesterolových steroidů byly vyšetřeny následující steroly: squalen a lathosterol (prekurzory syntézy cholesterolu) a betasitosterol a cambesterol (steroidy rostlinného původu). Nebyly zjištěny rozdíly v hladinách uvedených sterolů mezi pacienty s Alzheimerovou chorobou a vaskulární demencí. Tento nález svědčí jednak o tom, že syntéza cholesterolu nemusí být u nemocných s Alzheimerovou chorobou ovlivněna (squalen a lathosterol), ale i o tom, že stravovací zvyklosti jsou zřejmě u obou skupin pacientů stejné (betasitosterol a cambesterol).  Nejvýznamnějším nálezem bylo zjištění, že nutriční intervence provedená formou sippingu (600 kcal denně) vedla ke zpomalení progrese mentálního postižení u Alzheimerovy choroby. Intervenční studie tohoto typu a rozsahu nebyla dosud v dostupné literatuře popsána, náš nález je v tomto smyslu unikátní. Výsledky našeho studia prokázaly, že nutriční podpora může zpomalit progresi demence u Alzheimerovy choroby, přičemž neovlivňuje přežívání nemocných. Tyto výsledky jsou významné zejména proto, že nutriční podpora by tak mohla zlepšit kvalitu života pacientů s Alzheimerovou chorobou, aniž by prodlužovala jejich život v terminální fázi.

Abstract:

Alzheimer s disease (AD) is a degenerative disease of the central nervous system, affecting at least 3% of the population worldwide and the fourth most frequent cause of death in the developed countries. The disease occurs most often after the 65 year of age; in persons of over 85 years of age the prevalence is estimated to 50%. AD is regularly connected with a significant and gradual body weight loss. In spite of many theories (decreased food intake, changes in physical activity, energy expenditure alterations and metabolic changes similar to chronic inflammation) the aetiology of undernutrition in AD patients is not fully explained. Sufficient data on intervention regarding the progression of AD are also not available. Results of our prospective randomised study showed, that nutritional intervention in form of liquid supplements significantly decreased the progression of the deterioration of the AD patients´ mental functions. Therefore, we suggest that dietary counselling and nutritional support in patients with AD is clinically important and reduces the impairment of mental functions. Rationale: AD is regularly connected with a significant and gradual body weight loss – approximately 40% of the AD patients lose 4% of their body weight a year. A body weight loss greater than 5% in a year is considered a bad prognostic indicator in AD patients Methods: We studied the effect of nutrition support in 100 patients with Alzheimer disease (diagnosed according to the ICD-108). The grade of dementia was measured using the MMSE10. The Alzheimer disease patients were randomly divided into two groups – one with nutrition supplements 600 kcal/day (proteins 24g /day, carbohydrates 74 g/day, lipids 23 g day, fibres 9 g/day) to their ordinary food. The control group continued consuming ordinary food without nutritional supplements. Results: Food intake those patients who received a supplementary liquid diet had a constantly higher energy as well as protein intake. Supplementary liquid diet averted the decrease in MMSE values in the group of AP patients However, no significant difference was found between the patients with and without nutritional support as far as survival analysis was concerned. Nutrition support in our study not only improves nutrient intake but also slows down the progress of dementia and probably improve the quality of life of AD patients without any effect on survival rate. Conclusions: We observed a slowing down of the deterioration of mental functions in ADpatients who received nutritional support. Therefore we suggest that the nutritional support can have a positive influence on the mental state of the patients with Alzheimer's disease and can slow down the progress of this disease.
