Factors that affect the use of Nursing Diagnoses in practice

Abstract:

Cíl: Cílem práce bylo zjistit, které faktory nejvíce ovlivňují používání ošetřovatelských diagnóz v praxi, jaké jsou názory všeobecných sester na jejich používání a které motivační prvky by vedly k jejich kvalitnějšímu využití v praxi. Metodika: Výzkumný vzorek pro kvalitativní šetření tvořilo 8 všeobecných registrovaných sester. Na základě polostrukturovaného rozhovoru byly fenomenologickou analýzou vyhodnoceny jednotlivé faktory, jejichž míra vlivu byla zjišťována pomocí kvantitativního šetření metodou dotazníku. Celkový počet respondentů byl 253 z 9 zdravotnických zařízení. Výsledky: Z celkově zjištěných 22 faktorů, vyplývajících z rozhovorů, nejvíce ovlivňují používání ošetřovatelských diagnóz tyto faktory: počet pacientů na jednu sestru, administrativní zátěž, syndrom vyhoření, nedostatek času a rozdíly v teorii a praxi. K názoru na ošetřovatelské diagnózy se vyjádřilo 189 sester, z toho 49 % sester s nimi nesouhlasí, považuje je za zbytečné a zatěžující, 29 % sester s nimi souhlasí, považují je za důležité, ale jejich používání by mělo přínos jen za jiných podmínek v práci, než jsou dosud. 23 % sester s nimi souhlasí a vidí v nich přínos ve své práci. Nejvíce by sestry ke kvalitnější práci s ošetřovatelskými diagnózami motivovalo více času na pacienta, více personálu, zvýšení finančního ohodnocení, snížení administrativy a sjednocení ošetřovatelských diagnóz. Závěr: Na používání ošetřovatelských diagnóz v klinické praxi má vliv mnoho faktorů, z nichž za nejvýznamnější sestry označily počet pacientů na 1 sestru, administrativní zátěž a syndrom vyhoření. Rozpoznání všech těchto faktorů ovlivňujících používání ošetřovatelských diagnóz v praxi a zlepšení institucionální podpory může ulehčit implementaci ošetřovatelské diagnostiky v praxi. Z toho důvodu jsou výsledky tohoto výzkumu stěžejní pro ošetřovatelskou praxi a je třeba hledat cesty k řešení těchto faktorů, které negativně ovlivňují používání ošetřovatelských diagnóz.

Abstract:

Objective: The aim of this study was to determine which factors most affect the use of nursing diagnoses in practice, what are the views of nurses in their use and incentives that would lead to a better quality of their use in practice. Methods: The sample for the qualitative survey consisted of 8 registered general nurses. The individual factors based on the phenomenological analysis from the structured interview were evaluated and the extent of influence was detected by a quantitative questionnaire survey method. The total number of respondents was 253 from 9 health facilities. Results: Of the 22 identified factors resulting from the interviews, the following factors are most affected by using the nursing diagnosis: number of patients per nurse, administrative burden, burnout, lack of time and differences in theory and practice. The opinion of the nursing diagnosis is expressed by 189 nurses, of which 49% of nurses disagree with them, believing them to be unnecessary and burdensome, 29% of nurses agree with them, consider them important, but their use should only benefit in other circumstances at work than by now. 23% of nurses agree with them and see them benefit in their work. Most nurses would be better motivated at work using the nursing diagnoses if they had: more time for patients, more staff, increase financial evaluation, reduction and unification of the administration of nursing diagnoses. Conclusion: The use of nursing diagnoses in clinical practice is affected by many factors, among them the most important identified by nurses are the number of patients to one nurse, administrative burden and burnout. Recognition of all these factors affecting the use of nursing diagnoses in practice and improvement of institutional support can facilitate the implementation of nursing diagnoses in practice. Therefore, the results of this research are crucial for nursing practice and it is important to search for ways to deal with those factors that negatively affect the use of nursing diagnoses in practice.
