Selected problems of the patient with cirrhosis

Abstract:

Cíl: Cílem bakalářské práce je pomocí kazuistiky zachytit průběh a následný vývoj nemoci u vybraného pacienta s diagnózou jaterní cirhóza od počátečních příznaků až po nynější stav nemocného a zároveň popsat subjektivní prožívání této choroby. Metodika: Empirická část práce je zpracována formou kazuistiky. Je rozdělena do dvou oddílů ? anamnézu, katamnézu. V anamnéze popisuji podrobné informace o zdravotním stavu a životě pacienta před diagnostikováním choroby, rovněž se zabývám jeho nynějšími problémy. Katamnéza je věnována popisu průběhu již diagnostikované nemoci. Také zde rozebírám prožívání choroby pacientem a zachycuji změny psychického stavu, která chronická nemoc způsobuje. Jako zdroj informací jsem využila metodu nestandardizovaného rozhovoru s pacientem a jeho družkou, pozorování pacienta. Tyto skutečnosti jsem dále doplnila o fakta získaná ze zdravotnické dokumentace. Využila jsem také informací, které mi byly poskytnuty zdravotnickými pracovníky interního oddělení. Výsledky: Výsledek práce ukazuje, že průběh a vývoj nemoci u pacienta s jaterní cirhózou nemusí být vždy v souladu s literaturou. I přestože byla u pacienta patrná nonkompliance s léčebným režimem po celou dobu trvání nemoci, byla zjištěna zátěž, která pacienta po fyzické, psychické a sociální stránce velmi negativně ovlivňuje. Závěr: Touto prací bych chtěla poukázat na problémy pacientů s jaterní cirhózou a tímto i apelovat na včasné varování před jejími, z pohledu pacienta i rodiny, velmi nepříjemnými problémy. Rovněž považuji za důležitou důkladnou edukaci pacientů s tímto onemocněním a podporu v psychické oblasti.

Abstract:

Aim: The aim of the bachelor work is to capture current and following illness development at a certain patient with liver cirrhosis diagnosis, from the first signs to the current condition and, at the same time, to describe psychical state of patient who is going through this disease. Methodology: The empirical part of this work is processed by form of casuistry. It is divided into two parts ? anamnesis and catamnesis. In the part of anamnesis I describe detailed information about patient´s health condition and life before the diagnosed of the disease. I deal with his current problems. Catamnesis is engaged in description of the progress of the diagnosed disease. I deal with going through the disease by the patient and monitor changes in his psychological conditions, caused by the chronic disease. The source of information I used nonstandardized interview with the patient and his partner and observation the patient. Next to these facts I used information from the patient´s health documentation and information provided by the staff of internal department as well. Results: Results of my work show, that development of liver cirrhosis disease do not have to necessarily be in harmony with literature. Despite patient´s noncompliance during the medical treatment and during the time of the disease, were found out a heavy physical, psychical and social problems, which very negatively influencing the patient. Conclusion: By this work I would like to highlight the problems of patients with this disease and appeal for early warning of very unpleasant consequences, from the patient and family viewpoint. I consider thorough patient education and psychical support is vital.
