Riabilitazione post intervento di revisione protesi di ginocchio: protocollo Brotzman e utilizzo del Kinesio Tape

Rehabilitation after knee replacement revision surgery: Brotzman´s protocol and use of Kinesio Tape.

Abstract:

antecedent failure or wear of the prosthesis. The complication of the first intervention, such as infections, traumas, a not adequate positioning of the prosthesis, an incorrect balance of the soft tissue, can threaten the adequate prosthesis working. The purpose of the intervention is to defeat the inflammatory hotbed through the graft of a new prosthesis well positioned and functional, to remove pain and recover the functional knee joint. The Problems that physiotherapist has to face during the rehabilitation are edema excess; this may cause to the patient remarkable difficulty and pain in articular and the muscular tonicity recovering. Than a "blocked"articulation caused, either by the intervention, more aggressive than a total prosthesis system, or by previus months to the intervention, in which the patient has lived with a very rigid and aching limb and it could set up some reward mechanisms. The study's aim is to demonstrate how the application of Kinesio Tape, associated with a rehabilitation according to protocol Brotzman can help the pain symptomatology improvement, the edema reduction, articular and the muscular tonicity recovering in shorter period. Patients have been divided in two groups, 10 people each one. The first group, called for convention Group A, followed the rehabilitation plan according to Brotzaman protocol, whose objectives are the articular recover, muscular strengthen, edema and infection check, recovery proprioceptive and knee stability. The second group, called group B, followed the same group A?s rehabilitation plan , but associated with the application of Kinesio Tape, with different aim for each week: reduction of edema, hamstrings inhibition and quadriceps facilitation. Patients have been followed during all the studying period, four weeks long. Every week, at the beginning of the physical therapy session, VAS, Range of Motion, in flexion and extension, and thigh circumference were been evaluated. The results are all positive. The patients treated with Kinesio Tape had a significantly reduction of pain and edema especially in the first two weeks. Thanks to decrease of pain and edema and thanks to application of Kinesio Tape with the technique of inhibition and facilitation, patients have reached almost a complete articular activity in a shorter period than Group A patients.

Abstract:

La revisione di protesi totale di ginocchio č un intervento che viene eseguito conseguentemente a un fallimento o un?usura della protesi antecedente. Le complicazione del primo intervento, quali infezioni, traumi, un posizionamento non adeguato della protesi, disequilibrio dei tessuti molli, vanno a minacciare l?adeguato funzionamento della protesi. Lo scopo dell?intervento č di debellare il focolaio flogistico tramite l?impianto di una nuova protesi ben posizionata e ben funzionante, cosi da eliminare la sintomatologia algica e il recupero funzionale dell?articolazione. Le problematiche che il fisioterapista deve affrontare durante la riabilitazione sono l?eccesso di edema, che causa al paziente una notevole difficoltŕ e dolore nel recuperare l?articolaritŕ e la tonicitŕ muscolare; un?articolazione ?bloccata? a causa sia dell?intervento, piů aggressivo di un impianto di protesi totale, sia dai mesi precedenti all?intervento in cui il paziente ha convissuto con un arto molto rigido, dolente, che ha instaurato dei meccanismi di compenso. L? obiettivo di questo studio mira a dimostrare come l?utilizzo del Kinesio Tape, associato a una riabilitazione secondo protocollo Brotzman possa aiutare il miglioramento della sintomatologia algica, la riduzione dell?edema e un recupero dell?articolaritŕ e di tonicitŕ muscolare in tempi minori. Sono stati divisi i pazienti in due gruppi, da 10 persone ognuno. Il primo gruppo, chiamato per convenzione gruppo A, ha seguito il piano riabilitativo secondo protocollo Brotzaman, i cui obiettivi sono il recupero dell?articolaritŕ, rinforzo muscolare, controllo dell?edema e dell?infezione, recupero propriocettivo e della stabilitŕ del ginocchio. Il secondo gruppo, chiamato gruppo B, ha seguito lo stesso piano riabilitativo del gruppo A, ma con l?applicazione del Kinesio Tape, con scopi diversi ogni settimana: riduzione dell?edema, inibizione ischiocrurali e facilitazione del muscolo quadricipite. I pazienti sono stati seguiti nell?arco di tutto lo studio, di durata quattro settimane. Settimanalmente, prima della seduta di fisioterapia veniva valutata la VAS, il Range of Motion, in flessione e estensione, e la circonferenza della coscia. Tutti i dati che sono stati raccolti sono stati esaminati secondo metodo ANOVA (analisi della varianza).
