Dynamics of development of antibiotic resistance of urinary tract pathogens

Abstract:

Diplomová práce se zabývala problematikou antibiotické rezistence nejčastějších patogenů způsobujících močové infekce. Kultivačním vyšetřením byl ve vzorcích pacientů zjištěn původce (či původci) infekce, bylo provedeno testování citlivosti k antibiotikům a následně vyhodnoceno, která antibiotika byla vhodná k účinné léčbě. U vybraných pacientů se také provádělo vyšetření metodou RT-PCR za účelem detekce sexuálně přenosných patogenů. Výsledky statistické analýzy z let 2013–2019 ukázaly, že vývoj rezistence se nejvýrazněji projevil u bakterií Streptococcus alfa. Během sledovaného období došlo k nárůstu rezistence k antibiotiku cefuroxim (z 0 % na 16 %), benzylpenicilin (z 0 % na 21 %) a ampicilin (z 0 % na 32 %). Další případy nárůstu rezistence byly zjištěny u bakterií Enterococcus spp. k antibiotiku ampicilin (z 0 % na 21 %) a u bakterií Streptococcus beta k antibiotiku benzylpenicilin (z 0 % na 60 %). Dále se sledovaly klinické příznaky pacientů a rozdíly mezi pohlavím ve výskytu patogenů. Hlavním přínosem této práce bylo zjištění, že k empirické léčbě infekcí močových cest je nejvhodnější užívat antibiotikum nitrofurantoin.

Abstract:

The diploma thesis was focused on the issue of antibiotic resistance of the most common pathogens causing urinary tract infections. Patient samples were tested by culture examination to determine the cause of the infection. Furthermore, antibiotic susceptibility testing was performed and subsequently evaluated which antibiotics were suitable for effective treatment. The method RT-PCR was performed in some patients to detect sexually transmitted pathogens. The results of the statistical analysis from 2013–2019 showed that the development of resistance was pronounced in Streptococcus alfa. During the study period resistance increased to cefuroxime (from 0 % to 16 %), benzylpenicillin (from 0 % to 21 %) and ampicillin (from 0 % to 32 %). Further cases of increased resistance have been found in Enterococcus spp. to the antibiotic ampicillin (from 0 % to 21 %) and in Streptococcus beta to the antibiotic benzylpenicillin (from 0 % to 60 %). The clinical signs of patients and gender differences in the incidence of pathogens were also monitored. The main result of the study was the finding that the antibiotic nitrofurantoin is the most suitable for the empirical treatment of urinary tract infections.
