Ototoxicity Induced by Cisplatin – Clinical Observation and Determination of Individual Sensitivity

Abstract:

Úvod. Je prokázáno, že pacienti léčení chemoterapeutikem cisplatinou vykazují značnou interindividuální variabilitu v projevech nežádoucího účinku tohoto léčiva – ototoxicitě. Předpokládá se, že podkladem této skutečnosti jsou genetické faktory. Cílem projektu bylo nalezení závislosti mezi výskytem ototoxicity indukované cisplatinou a variabilitou v genech. Metody. V prospektivní studii bylo analyzováno 55 pacientů léčených cisplatinou, u kterých se nevyskytovalo významné poškození sluchu v předchorobí. Pacienti byli audiometricky vyšetřeni před terapií a následně před a po každém cyklu podání cisplatiny. Byly zkoumány genové polymorfismy ve vybraných kandidátních genech, kódujících enzymy ovlivňující oxidativní stres, mediátory zánětu , regulaci apoptózy a methyltransferázy. Výsledky. V souboru vyšetřených pacientů byly prokázány asociace časného nástupu ototoxicity indukované cisplatinou s nepřítomností delece genu pro GSTT1 (p=0,009) a s výskytem T alely polymorfismu rs9332377 v genu pro COMT (p=0,001). Byly také zjištěny rozdíly v senzitivitě audiologických metod sledování ototoxicity. Závěr. Pomocí prospektivní studie byl podán důkaz o genetickém podkladu individuální senzitivity k ototoxicitě indukované cisplatinou. Časný nástup ototoxicity byl pravděpodobnější u jedinců bez delece genu pro GSTT1 a s T alelou polymorfismu rs9332377.

Abstract:

Background. It is clearly demonstrated that patients treated by chemotherapy with cisplatin show considerable interindividual variability in the manifestation of ototoxicity. It is believed that genetic factors are the basis of this fact. The aim of the project was to find dependence between the occurrence of ototoxicity induced by cisplatin and variability in the genes. Methods. A group of 55 patients was analyzed in a prospective study. These patients were treated by cisplatin and did not reveal significant hearing loss in personal history. All patients were examined by audiometry prior to therapy and then before and after each cycle of cisplatin. Genetic polymorphisms were investigated in selected candidate genes encoding enzymes involved in oxidative stress, inflammatory mediators, regulation of apoptosis and methyltransferase. Results. In the study group was demonstrated association of early onset cisplatin-induced ototoxicity with the absence of gene deletion for GSTT1 (p = 0.009) and the occurrence of the T allele of rs9332377 polymorphism in the COMT gene (p = 0.001). There were also found differences in sensitivity of audiological methods used for monitoring ototoxicity. Conclusions. The prospective study has brought a clear evidence that the individual sensitivity to cisplatin-induced ototoxicity is influenced by genetic background. Early onset of ototoxicity was more probably in patients without gene deletion for GSTT1 and with T alelle of rs9332377 polymorphism.
