Nutrition of the elderly at hospital admission

Abstract:

Cíl: Cílem diplomové práce bylo zjistit a zhodnotit stav výživy seniorů přicházejících k hospitalizaci, zmapovat četnost výskytu malnutrice a nalézt vztahy mezi jednotlivými faktory, které úroveň výživy seniorů ovlivňují. Metodika: Výzkum má charakter kvantitativního šetření. Ke sběru dat byly použity standardizované dotazníky - MNA - SF (Mini Nutritional Assessement - Short Form), ADL (Activities of Daily Living), MMSE (Mini Mental State Examination). Výzkumné šetření probíhalo od července do září 2016. Výzkumný soubor tvořilo 169 seniorů. Výsledky: Výzkumným šetřením jsme prokázali, že nutriční stav, úroveň soběstačnosti a stav kognitivních funkcí se vzájemně ovlivňují. Výskyt poruch výživy se zvyšoval s klesající úrovní soběstačnosti ? největší podíl osob s podvýživou (63 %) byl ve skupině vyjadřující vysokou závislost. Také kognitivní dysfunkce byly spojeny s vyšší prevalencí podvýživy a výsledky výzkumu prokázaly pozitivní korelaci mezi hodnotou MMSE a MNA ? SF (p <0,001, r ? 0,6536). V době přijetí k hospitalizaci bylo v našem souboru klasifikováno v riziku podvýživy 40 % seniorů. V hodnocení stavu výživy mezi muži a ženami (p ? 0,051) a ve vztahu k věku nebyly prokázány signifikantní rozdíly (p ? 0,820). Závěr: Hodnocení stavu výživy seniorů a identifikace rizikových faktorů jsou důležitou součástí posuzování celkového stavu seniorů při přijetí k hospitalizaci. Rozpoznání deficitů v jednotlivých oblastech a adekvátní plánování intervencí a jejich následná realizace mohou zabránit v dalším zhoršování nejen nutričního stavu seniorů. Pokud by nebyly poruchy výživy včas rozpoznány a léčeny, hrozí řada komplikací zdravotního stavu, prodloužení délky hospitalizace i zhoršení soběstačnosti seniorů.

Abstract:

Aim: The aim of the thesis was to identify and evaluate the nutritional status of the elderly coming to the hospital, to map the the incidence of malnutrition and find relationships between individual factors that influence the the level of their nutrition. Methods: Quantitative research methods were used. Data were collected by stan-dardized questionnaires - MNA - SF (Mini Nutritional Assessement - Short Form), ADL (Activities of daily living), MMSE (Mini Mental State Examination). The research was carried out from July to September 2016. Research sample consisted of 169 elderlies. Results: The research results show that nutritional status, level of self-sufficiency and conditions of cognitive functions influence each other. Incidence of nutrition disorders increase with decreasing levels of self-sufficiency - the largest proportion of people (63%) were undernourished in the group expressing high dependence. Cognitive dysfunction were associated with a higher prevalence of malnutrition and research results have shown a positive correlation between the MMSE and the MNA - SF (p <0,001, r ? 0,6536). At the time of admission to hospital was in our group classified at risk of malnutrition 40 % of elderly. There were no significant correlations between men and women (p ? 0,051) and age (p ? 0,820). Conclusion: The nutritional status of elderly and identification of risk factors are an important part of the assessment of the overall condition of elderly at hospital admission. Recognition of deficits in individual areas, adequate planning of interventions and their subsequent implementation might prevent further deterioration of the nutritional status not only at elderly. If the eating disorders were not recognized and treated timely, it may threaten the health status, prolong hospitalization and worsen the self-sufficiency of elderly.
