The comparision of physiotherapy with and without using the Vojta therapy by the patients after the stroke

Abstract:

Cíl: Mým cílem je porovnat efekt využití Vojtovy metody u pacientů po CMP oproti klasickým terapeutickým postupům, které se užívají. Dále jsem chtěla zjistit, zda zařazení reflexní lokomoce dle Vojty do terapie po delší časový úsek (4 týdny) bude mít stále efekt nebo zda tento efekt odezní například po pár dnech. Metodika: Metodou je případová studie na dvou konkrétních pacientech, které jsem si vybrala ve dvou obdobích během svých praxí v chomutovské nemocnici na oddělení neurologie. Ti poté měsíc na základě vstupního kineziologického rozboru každodenně podstupovali fyzioterapeutickou intervenci a to dvakrát denně a prováděli autoterapii. U obou pacientů jsem aplikovala klasické fyzioterapeutické postupy jako cévní a dechová gymnastika, míčkování, PNF, prvky z Bobath konceptu atd. U druhého pacienta jsem navíc doplnila terapii o Vojtovu reflexní lokomoci. Na závěr jsem porovnala výsledky výstupních kineziologických rozborů a zhodnotila je. Výsledky: Zjištěním je, že hybnost u pacienta s Vojtovou metodou se zlepšovala velmi výrazně oproti druhé pacientce a vždy po Vojtově terapii byla vidět aktivní samovolná hybnost, která byla o mnoho lepší než před terapií, zlepšení bylo pozorovatelné hlavně u extenze zápěstí a prstů a také u dorzální flexe, zaktivizovalo se brániční dýchání a aktivoval trup. V závěru lze říci, že zařazení Vojtovy metody u dospělých pacientů, ještě více u pacientů po CMP, má svůj potenciál a opodstatnění, které lze podtrhnout rychlejší vertikalizací, zlepšením hybnosti HK i DK a aktivací HSSp.

Abstract:

Purpose: My goal is to compare the effect of using Vojta method by the patients after stroke compared to classical therapies that are used. I also wanted to find out whether the inclusion of Vojta reflex locomotion into therapy for a longer period of time (4 weeks) would still have an effect or whether this effect would disappear after a few days, f.e. Methodology: The method is a case study by two specific patienst, which I chose between two periods during my internship at the Chomutov hospital in the department of neurology. Then they underwent a daily physiotherapeutic intervention on the basis of the initial kinesiological analysis twice a day and performed autotherapy. I applied by both patients classical physiotherapeutic procedures such as vascular and respiratory gymnastics, balling (massage by soft ball), PNF, elements from the Bobath concept etc. By the second patient I also supplemented the therapy with Vojta´s reflex locomotion. Finally, I compared the results of the output kinesiological analysis and evaluated them. Results: The finding is that the movement by the patient with Vojta method (compared to the second patient) was improved very significantly and always after Vojta therapy was seen active spontaneous movement, which was much better than before therapy, improvement was observed mainly in the extension of wrists and fingers left arm and also in dorsal flexion left leg, then activated diaphragm breathing and activated the trunk. In conclusion, the inclusion of the Vojta method by adult patients, even more by patients after stroke, has its potential and justification, which can be underlined by faster verticalization, improved mobility of upper and lower limb and activation of DSSS.
