Abstract:

Cílem této práce bylo popsat problémy komplexní péče o chrup u pokročilé agresívní parodontitidy a specifikovat je. Jako metodika byla zvolena kazuistika pacientky s dlouhodobou diagnostikou tohoto onemocnění, která popisuje komplexní péči o chrup od jejího počátku, v jednotlivých fázích léčby, včetně recidivy s částečnou ztrátou zubů, až do současnosti. Popisuje problémy ústní hygieny, která nebyla před zahájením léčby u pacientky optimální a efektivní. Edukací bylo dosaženo zefektivnění ústní hygieny, kterou si pacientka osvojila a udržela správné hygienické návyky v péči o chrup dosud. Provádění ústní hygieny bylo nesnadné po chirurgických zákrocích, v období progrese a recidivy onemocnění i v době řešení pevných protetických náhrad po částečné ztrátě zubů. Časově náročná léčba, zejména její chirurgická fáze, která onemocnění stabilizovala, byla příčinou kontrakcí dásní a obnažených hypersenzitivních krčků zubů. Jejich ošetření si vyžádalo další zákroky. Období přípravy chrupu pro usazení pevných protetických náhrad pacientce přechodně způsobilo fonetický problém a společenský handicap. Teoretická část práce je zaměřena na symptomatologii, diagnostiku a léčbu parodontitidy. Popisuje metody prevence onemocnění, edukaci ve stomatologické péči, roli sestry v edukaci a dentální hygienu. Kazuistika byla rozdělena do jednotlivých částí a obsahuje anamnézu, katamnézu, analýzu a interpretaci, diskusi a závěr.

Abstract:

The main purpose of this work was to describe the problems of the complex teeth care in developed aggressive periodontitis and to specify them. As the method was choosen the casuistry of the woman with long term diagnosis of this kind of the disease which describe the complex dental care from the beginning, each individual stage of the therapy, including the recidivism with partial dental loss, up till now. It also describe the problems of oral care which wasn´t optimal and effective enought before the beggining of the treatment. By proper education of the patient the dental care was developed to be more effective and was put in to the practice up til now. Maintaining the proper oral care was not an easy after the surgeries, during the period of the progression and recidivism of the dissease, neither during the time of choosing a fixed dental bridge after partial dental loss. The time consuming therapy, mainly its surgical part which stabilized the dissease, caused the contracture of the gingivae and exposed hypersenzitive tooth root. Their treatment required another intervention. The period of the dental preparation for placement of the fixed prothetic dental bridge caused temporary phonetic problem and social handicap to the patient. The theoretical part of the work is focused on the symptomatology, diagnosis and the therapy of the periodontitis. It describes the prevention methods of the disease, education in dental care, the role of the nurse during the education and dental hygiene. The casuistry was divided into few individual parts and contains the anamnesis, analysis, interpretation, discussion and the conclusion.
