Non-invasive approaches for monitoring organ rejection after transplantation

Abstract:

Protože orgánová rejekce představuje závažnou zdravotní komplikaci během potransplantačního období a může vést ke sníženému přežití štěpu, je důležité vyvinout spolehlivé metody monitorování pacientova stavu. V tomto světle se jeví detekce hladin volné cirkulující DNA (cfDNA) jako výjimečná metoda. Práce představuje krátké shrnutí invazivních i neinvazivních metod, jež jsou používány ke sledování rejekcí, a obsahuje rovněž příklady postupů, které jsou v současné době ve stádiu zkoumání. Důraz je kladen na biomarkery, jmenovitě na aktuální přehled poznatků týkajících se cfDNA. Hlavním cílem práce je zhodnotit potenciál cfDNA jako markeru rejekce. Na základě shromážděných údajů tento text přispívá k obecné orientaci v inovativních metodách diagnostiky odvržení transplantátu.

Abstract:

Since transplant rejection embodies a major health complication during the post-transplantation period and can, therefore, lead to decreased allograft survival, it is necessary to develop reliable methods for organ monitoring. In this context, non-invasive surveillance by evaluating the levels of cell-free DNA seems to be an outstanding approach. The thesis presents a brief summary of both invasive and non-invasive methods used in rejection monitoring and contains examples of currently examined approaches for transplant surveillance as well. The possible use of non-invasive biomarkers is highly emphasized. An accent is put particularly on up-to-date overview of cell-free DNA which is based on already published studies concerning this issue. The very aim of the thesis is to assess the potential of cell-free DNA as a marker of rejection. On the grounds of the gathered findings, this thesis contributes to general orientation in the novel methods of rejection diagnostics.
