Fekální bakterioterapie v léčbě rekurentní klostridiové kolitidy

Fecal bacteriotherapy in the treatment of recurrent Clostridium difficile infection

Abstract:

Transplantace střevní mikrobioty představuje unikátní terapeutickou modalitu, jež má za cíl obnovit přirozenou diverzitu intestinální mikrobioty a tím zabránit rekurencím jedné z nejvýznamnějších nozokomiálních infekcí současné doby – klostridiové kolitidy. Cílem prospektivní observační monocentrické studie bylo posoudit účinnost a bezpečnost fekální bakterioterapie v léčbě klostridiové kolitidy na pracovišti, které v rámci České republiky provedlo tento výkon jako první již v roce 2010 a jež je stále republikovým leaderem v počtu realizovaných fekálních transplantací. Pacienti s klostridiovou kolitidou byli primárně léčeni cílenou antibiotickou (ATB) terapií, transplantace střevní mikrobioty jim byla aplikována v rámci sekundární profylaxe po ukončení ATB terapie. Pokud do osmi týdnů po výkonu nedošlo u pacienta k návratu onemocnění, byla léčba hodnocena jako úspěšná. Kromě celkového počtu vyléčených pacientů nás zajímalo, zda některý z faktorů, které charakterizují léčené pacienty, má statisticky významný vliv na hodnocení úspěšnosti léčby. Mezi posuzované faktory byly zařazeny: věk a pohlaví, počet předchozích epizod onemocnění, závažnost léčené kolitidy, přítomnost chronických komorbidit, performance status, iniciální ATB léčba, způsob aplikace fekálního transplantátu a použití čerstvé nebo zamražené stolice dárce. K posouzení závislosti úspěšnosti léčby na uvedených faktorech byla použita jednorozměrná statistická analýza a následně logistická regrese. Ve sledovaném intervalu čtyř let (2015–2018) bylo pomocí transplantace střevní mikrobioty léčeno 172 pacientů. Úspěšnost ve studovaném období činila v rámci primární odpovědi na léčbu 77,4 %. Analýzou podskupin byly jako rizikové faktory selhání léčby identifikovány vyšší věk pacientů, vyšší Charlson Comorbidity Index, odrážející přítomnost a závažnost dlouhodobých komorbid a vyšší performance skóre, které odráží funkční stav pacientů. Ve sledovaném období byly pozorovány dvě závažné nežádoucí události, v obou případech se jednalo o perforaci stěny konečníku, vzniklou periprocedurálně při aplikaci suspenze stolice klyzmatem. Letalita nebyla zaznamenána. Transplantace střevní mikrobioty je efektivní a bezpečnou terapeutickou alternativou pro pacienty s rekurentní kolitidou vyvolanou Clostridioides difficile.

Abstract:

Faecal microbiota transplantation represents a unique therapeutic modality targeted to restore the natural diversity of intestinal microbiota and to prevent recurrence of one of the most significant nosocomial infections of the present time – Clostridioides difficile infection (CDI). The aim of this prospective observational single-center study was to assess the success rate and safety of faecal bacteriotherapy in the treatment of CDI at a clinic that had been the first in the Czech Republic to perform this procedure as early as 2010, and still is the national leader in the number of realized transplantations to-date. Patients with CDI were primarily treated with a targeted antibiotic (ATB) therapy; they received the faecal microbiota transplantation as a secondary prophylaxis after completion of the ATB therapy. If colitis did not recur within eight weeks after the procedure, the treatment was evaluated as successful. Aside of the total number of cured patients, we were interested in finding out if any of the factors used to categorize the treated patients had statistically significant influence on the success rate of this therapy. The assessed factors included: age and sex, the number of previous disease episodes, the severity of the treated colitis, the presence of chronic comorbidities, the performance status, the initial ATB treatment, the way of faecal transplantation application and the use of fresh or frozen donor stool. Unidimensional statistical analysis followed by logistic regression were used to assess the dependence of treatment success on the listed factors. Within the monitored four-year interval (2015–2018), 172 patients were treated by means of faecal microbiota transplantation. The efficacy of primary response to treatment in the study period was 77.4 %. Subgroup analysis identified the following risk factors for treatment failure: higher patient age, higher Charlson Comorbidity Index (reflecting the presence and severity of long-term comorbidities) and higher performance score (reflecting the patients’ functional state). Two serious adverse events were observed in the study period; both cases were of rectal wall perforation, and occurred during stool suspension application by an enema. No lethality was registered. Faecal bacteriotherapy is a successful and safe therapeutic alternative for recurrent colitis caused by Clostridioides difficile.
