Abstract:

Úvod: Astma bronchiale je chronické zánětlivé onemocnění, které bychom mohli považovat za onemocnění dnešní doby. Jeho incidence během posledních let v České republice roste. Mezi primárně zvolenou metodu v komplexní terapii s astma bronchiale řadíme respirační fyzioterapii. V této bakalářské práci se zaměřuji na problematiku respirační fyzioterapie v souvislosti s chronickým onemocněním astma bronchiale. Cíl práce: Cílem bakalářské práce je přispět k řešení problematiky respirační fyzioterapie u pacienta s astma bronchiale. Metodika: Praktická část mé bakalářské práce je kazuistika 23letého pacienta s onemocněním astma bronchiale. Součástí je odebrání anamnézy s následným provedením kineziologického rozboru. Na základě výsledků získaných ze vstupního vyšetření byl stanoven krátkodobý fyzioterapeutický plán. Fyzioterapeutické intervence probíhaly jednou až dvakrát během čtrnácti dní v délce 60 minut v období od 29. 1. 2023 do 24. 8. 2023. Během terapie byly využity techniky měkkých tkání, postizometrická relaxace, postfacilitační strečink, mobilizace, dynamická neuromuskulární stabilizace a techniky respirační fyzioterapie. Po skončení terapie bylo provedeno výstupní vyšetření a stanoven dlouhodobý fyzioterapeutický plán. Výsledky: Vliv terapie lze pozorovat ve spirometrickém vyšetření, kde shledáváme pozitivní efekt ve zvýšení celkové plicní kapacity, inspirační kapacity, usilovné vitální kapacity, usilovně vydechnutého objemu za sekundu, a dalších parametrů. Dále dle získaných výsledků z dotazníku kontroly astmatu pozorujeme lepší kontrolu nad onemocněním a přesunu pacienta do kategorie, kdy má astma pod kontrolou. Dle subjektivního hodnocení pacienta došlo ke zmírnění symptomů onemocnění a zmenšení bolesti. Došlo ke zlepšení dechovém stereotypu, u kterého lze pozorovat začátek v dutině břišní s postupným rozvíjením do krajiny hrudní a dále pozorujeme nádech nosem a výdech ústy. Závěr: Terapii shledávám z větší části úspěšnou. Prohloubila jsem si znalosti o dané problematice a získala nové zkušenosti s užitečnými vědomostmi, které v budoucnu využiji ve své praxi.

Abstract:

Introduction: Asthma bronchiale is a chronic inflammatory disease that we could consider a disease of our time. Its incidence has been increasing in the Czech Republic in recent years. Respiratory physiotherapy is among the primarily chosen methods in complex therapy with bronchial asthma. In this bachelor's thesis, I focus on the issue of respiratory physiotherapy in connection with the chronic disease asthma bronchiale. Aim: The aim of the bachelor's thesis is to contribute to solving the problem of respiratory physiotherapy in a patient with bronchial asthma. Methodology: The practical part of my bachelor thesis is a case study of a 23-year-old patient with bronchial asthma. Part of it is taking an anamnesis followed by a kinesiological analysis. Based on the results obtained from the initial examination, a short-term physiotherapy plan was established. Physiotherapy interventions took place once or twice during a fortnight lasting 60 minutes in the period from 29. 01. 2023 to 24. 08. 2023. Soft tissue techniques, post-isometric relaxation, post-facilitation stretching, mobilization, dynamic neuromuscular stabilization were used during the therapy and respiratory physiotherapy techniques. After the end of the therapy, an exit examination was performed and a long-term physiotherapy plan was established. Results: The effect of the therapy can be observed in the spirometric examination, where we find a positive effect in increasing the total lung capacity, inspiratory capacity, effortful vital capacity, effortfully exhaled volume per second, and other parameters. Furthermore, according to the results obtained from the asthma control questionnaire, we observe a better control over the disease and the transfer of the patient to the category where the asthma is under control. According to the subjective evaluation of the patient, the symptoms of the disease have decreased and the pain has decreased. There has been an improvement in the breathing stereotype, which can be observed starting in the abdominal cavity with gradual development into the thoracic landscape, and we further observe inhaling through the nose and exhaling through the mouth. Conclusion: I find the therapy successful for the most part. I deepened my knowledge of the given issue and gained new experience with useful knowledge that I will use in my practice in the future.
