Analýza zánětlivého procesu u pacientů s autoimunitními revmatickými onemocněními: Role proteinů tepelného šoku 90kDa u revmatoidní artritidy a axiálí spondylyartritidy

Abstract:

Porušení rovnováhy imunitního systému ztrátou autotolerance je jednou z příčin rozvoje chronického zánětlivého procesu u revmatoidní artritidy. Jedním z potenciálních účastníků tohoto děje jsou i proteiny tepelného šoku. Jsou součástí všech procesů imunitní odpovědi –fáze rozpoznání, imunologické odpovědi i následné regulace této odpovědi a ukončení zánětlivého procesu. Cílem práce bylo stanovit hladiny proteinu tepelného šoku 90kDa ( Hsp90k) v séru pacientů s revmatoidní artritidou (RA) a axiální spondylartritidou (AxSpA) a zhodnotit korelaci těchto nálezů se zánětlivou aktivitou onemocnění. V hodnocených souborech byla prokázána nevýznamně vyšší hladina Hsp90 u pacientů s RA při srovnání se skupinou zdravých dobrovolníků (p=0,10). Ve skupině pacientů s AxSpA nebyl rozdíl v hladinách Hsp90 (p=0,39) oproti zdravým kontrolám. Byl prokázán vliv biologické terapie anti-TNFα inhibitory na hladiny Hsp90 u RA; ve skupině pacientů s aktuální terapií některým z anti-TNFα preparátů byly hladiny Hsp90 signifikantně nižší (p=0,015). Taktéž jsme pokázali korelaci vyšších hladin Hsp90 s laboratorními parametry aktivity onemocnění v souboru RA ( korelace hladin CRP a Hsp90, p= 0,016), v souboru pacientů s AxSpA nebyla tato korelace zjevná. Závěr. Výsledky naší práce poukazují na možný význam Hsp90 na regulaci zánětlivého procesu u revmatoidní artritidy. Data získaná z preklinických zvířecích modelů artritidy ukazují na možnost ovlivnění zánětlivého procesu u RA prostřednictvím modulace některých Hsp. Prohloubení těchto vědomostí a pochopení vzájemných vztahů Hsp a ostatních činitelů autoimunitního procesu by mohlo definovat nové terapeutické cíle a vést k poznání mechanismu znovunastolení autotolerance.

Abstract:

Unbalancing of immune system due to loss of self-tolerance is one reason for the development of the chronic inflammatory process in rheumatoid arthritis. One of the potential participants of this process are heat shock proteins. They influences each phase of the immunological event – recognition, immune response and subsequent regulation of the inflammatory response and resolution. The aim of this study was to determine the level of heat shock protein 90kDa (Hsp90k) in the serum of patients with rheumatoid arthritis (RA) and axial spondylarthritis (AxSPA) and to evaluate the correlation of these findings with inflammatory activity of diseases. We demonstrated statistically insignificant higher levels of Hsp90 in patients with RA when compared with healthy volunteers (p = 0,10). In the group of patients with AxSPA was no difference in the levels of Hsp90 (p = 0.39) compared with healthy controls. We demonstrated the effect of the biological treatment with anti TNF inhibitors on Hsp90 levels in RA patients, in the group with current anti TNF therapies Hsp90 levels were significantly lower (p = p=0,015). Moreover Hsp90 levels in RA patients correlate with laboratory parameters of disease activity (correlation between levels of CRP and Hsp90, p = 0,016). In the AxSpA group there was no correlation with disease acitivty. Conclusion. Our results point to the potential significance of Hsp90 in the regulation of inflammation in rheumatoid arthritis. Data obtained from preclinical animal models of arthritis suggest the possibility of influencing the inflammatory process in RA via modulation of some Hsp. Deepening the knowledge and understanding of the interrelationships between Hsp and other factors of autoimmune process could define new therapeutic targets and lead to the understanding of self-tolerance restoring mechanism.
