Recovery of post-stroke fatigue: indications for neurological rehabilitation

Abstract:

Many stroke patients report that they experience enormous fatigue during the day and that they get completely exhausted in the evening, due to the physical and mental load they are subjected to following the disease. All cognitive abilities reside in the brain and any alteration of the biology of the nervous system changes these abilities negatively. Often, even the family members of a stroke survivor can not understand and become accustomed to his extreme propensity to get tired during the day and attribute this condition simply to laziness or unwillingness. In reality we must imagine the immersed effort that a hemiplegic patient is having to face in his daily life from the first moments of the morning, motor difficulties and daily challenges with a lateral half of the body forced to movements reduced or often absent or altered from spasticity, one of the first post-stroke "enemies", understood as stiffening of the muscles and involuntary contractions under stress. Therefore, having a rigid and heavy part of the body exposes the patient to a greater effort than a healthy subject that requires a greater reflex contraction of the muscles which in turn will make the movement harder and heavier, to this must be added cognitive difficulties such as capacity for attention and concentration that strain even more the mind and the body.

Abstract:

Molti pazienti colpiti da ictus riferiscono di provare un?enorme stanchezza durante la giornata e di arrivare alla sera completamente sfiniti, ciň č dovuto al carico di impegno fisico e mentale a cui sono sottoposti in seguito alla malattia. Tutte le abilitŕ cognitive risiedono nel cervello e qualsiasi alterazione della biologia del sistema nervoso modifica in negativo queste abilitŕ. Spesso anche i familiari di un paziente sopravvissuto ad un ictus non riescono a capacitarsi ed abituarsi alla sua estrema propensione ad affaticarsi durante la giornata ed attribuiscono tale condizione semplicemente alla pigrizia o mancanza di volontŕ. In realtŕ bisogna immaginare l?immerso sforzo che un paziente emiplegico si trova a dover affrontare nella sua quotidianitŕ sin dai primi istanti del mattino, la difficoltŕ motoria e le sfide quotidiane con una metŕ laterale del corpo costretta a dei movimenti ridotti o spesso assenti o alterati dalla spasticitŕ, uno dei primi ?nemici? post stroke ,intesa come irrigidimento dei muscoli e contrazioni involontarie sotto sforzo. Dunque avere una parte del corpo rigida e pesante espone il paziente ad uno sforzo maggiore rispetto a un soggetto sano che necessita di una maggiore contrazione riflessa dei muscoli che a sua volta renderŕ piů duro e pesante il movimento, a questo vanno aggiunte le difficoltŕ cognitive quali capacitŕ di attenzione e concentrazione che affaticano ancor di piů la mente ed il corpo.
