Abstract:

Anestezie a analgezie v peripartálním období je nedílnou součástí péče o rodičku v zemích s vyspělým zdravotnickým systémem. Tato dizertační práce předkládá souhrnné informace o anesteziologických a algeziologických aspektech péče v tomto specifickém období ženy na podkladě aktuálně publikovaných zjištění. Cílem práce bylo zmapovat oblast porodnické anestezie a analgezie na pracovišti autora a porovnat je se zavedenou praxí v České republice. Dalším cílem bylo srovnat moderní metodu tlumení porodní bolesti remifentanilem se standardní metodou v podobě epidurální analgezie bupivakainem se sufentanilem. Dále bylo cílem zjistit, jaký dopad na úroveň tlumení pooperační bolesti po císařském řezu bude mít zavedení institucionalizovaného Acute Pain Service a jak ovlivňuje úroveň pooperační bolesti zvolený typ anestezie a forma pooperační analgezie. Posledním cílem předkládané práce bylo zjistit, zda aplikací kombinované neopioidní analgezie lze dosáhnout srovnatelné míry analgezie po císařském řezu vedeném v celkové anestezii. Anesteziologická praxe na našem pracovišti se od zavedené praxe v rámci 49 zapojených pracovišť v České republice liší vyšší četností užití epidurální analgezie, možností podání remifentanilu k porodní analgezii, vyšší četností použití epidurální anestezie s katétrem u císařského řezu a možností využívat zavedený epidurální katétr k pooperační analgezii. Nízká četnost polohování rodičky s levostrannou dislokací dělohy (15º) skýtá prostor pro edukaci důležitosti toho jednoduchého postupu sloužícího pro prevenci aorto-kaválního syndromu, který není rozšířen ani celorepublikově. Rodičkou řízené intravenózní podání remifentanilu i epidurální analgezie poskytují účinnou analgezii v průběhu porodu s vysokou spokojeností rodičky a dobrým klinickým novorozeneckým výstupem. Prezentovaná data podporují klinické použití rPCA jako možné alternativy ke standardní epidurální analgezii. Nejúčinnější metodou analgezie po císařském řezu je kontinuální epidurální blokáda. Není-li epidurální katétr zaveden, není třeba jej dále zavádět, systémová analgezie poskytuje klinicky dobrou úlevu od bolesti. Počet přídavných analgetických požadavků signifikantně klesá po 24 hod. od konce císařského řezu u všech metod pooperační analgezie, proto zavedení epidurálního katétru delší než 24 hodin či jen pro pooperační analgezii po SC nemá opodstatnění. Zavedení Acute Pain Service po císařském řezu vede k poklesu průměrného VAS do 12 hodin po operačním výkonu při srovnání skupin před a po zavedení servisu. Aplikací opioidní i neopioidní analgezie po císařském řezu vedeném v celkové anestezii lze dosáhnout bezpečně dostatečného analgetického efektu v prvních 24 hodinách pooperačně. Účinnější je podání opioidní analgezie po císařském řezu. Závěrem dizertační práce je doporučený postup pro vedení anestezie a analgezie pro pacientky Gynekologicko-porodnické kliniky Fakultní nemocnice Brno – část Bohunice.

Abstract:

Anaesthesia and analgesia in the peripartum period is an integral part of the care for a woman in developed countries. This thesis presents a summary of anaesthesiological and algesiological aspects of the care in this specific women‘s period on the basis of currently published findings. The aim was to map the field of obstetric analgesia and anaesthesia on the author’s workplace and compare the findings with the established practice in the Czech Republic. Another objective was to compare the modern method of labour analgesia with remifentanil to the standard method in the form of epidural bupivacaine analgesia with sufentanil. Furthermore, the aim was to determine the impact on the postoperative pain control after caesarean section will be the introduction of an institutionalized Acute Pain Service and how it affects the level of postoperative pain selected type of anaesthesia and postoperative analgesia form. The final aim of this study was to determine whether the application of combined non-opioid analgesia can achieve a comparable level of analgesia after caesarean section under general anesthesia. Anaesthesia practice in our department differs from the established practice of the 49 participating centers in the Czech Republic in an increased use of epidural analgesia, a remifentanil administration option for obstetric analgesia, a higher frequency of use of epidural anaesthesia with catheter for caesarean section and the possibility to use this epidural catheter for postoperative analgesia. Low frequency of maternal positioning with left-dislocation of the uterus (15 º) provides space for education, the importance of serving a simple procedure for the prevention of aorto-caval syndrome, which is not wide-spread Woman controlled intravenous remifentanil administration and epidural analgesia provide effective analgesia during the labor with high maternal satisfaction and good clinical neonatal outcome. The data presented here support the clinical use of rPCA as the possible alternative to standard epidural analgesia. The most effective method of analgesia after caesarean section is a continuous epidural blockade. When the epidural catheter is not inserted in, there is no need to place it, systemic analgesia provides clinically good pain relief. Additional analgesic requests number significantly decreases after 24 hours from the end of caesarean section with all methods of postoperative analgesia, hence the introduction of epidural catheter longer than 24 hours or just for postoperative analgesia after SC has no merit. Introduction of Acute Pain Service after caesarean section leads to a decrease in the average VAS within 12 hours after surgery when comparing groups before and after the service. Application of opioid and non-opioid analgesia after caesarean section administered under general anaesthesia can be achieved safely sufficient analgesic effect in the first 24 hours postoperatively. More efficient is the administration of opioid analgesia after caesarean section. Finally, this dissertation thesis is the recommended procedure for the conduct of anaesthesia and analgesia for patients of Department of Gynaecology and Obstetrics, University Hospital Brno - Bohunice part.
