Abstract:

ABSTRAKT Cíl: Cílem diplomové práce bylo zjistit schopnost sebepéče dětí s onemocněním diabetes mellitus 1.typu. Dále zaznamenat vybrané faktory, které ovlivňují schopnost sebepéče dětí a úroveň jejich znalostí ve vybraných oblastech sebepéče. Metodika: Výzkumný soubor tvořilo 101 pacientů dětské endokrinologické ambulance Městské nemocnice Ostrava a endokrinologické a dětské diabetologické ambulance Medica Iberia s.r.o. v Opavě ve věku 6-18 let. Dotazníkové šetření probíhalo od 11. 12. 2017 do 11. 2. 2018. Ke sběru dat byl použit modifikovaný dotazník The Confidence in Diabetes Self-care Scale (CIDS), zjišťující schopnost sebepéče dětí s diabetem. Pro popis souboru byla použita popisná statistika. Pro vyhodnocení hypotéz byl použit Kruskal-Wallisův test a Spearmanův korelační koeficient. Výsledky: Šetřením bylo zjištěno, že celková úroveň sebepéče není závislá na věku dítěte, avšak některé schopnosti a dovednosti mohou být věkem ovlivněny. Děti v nejnižší věkové skupině nejhůře zvládají úpravy inzulínového režimu, a tato schopnost se zdokonaluje s věkem. Děti v nejstarší skupině jsou si v úpravách inzulínového režimu nejjistější. Naopak nejmladší děti jsou nejvíce svědomité v denním záznamu hodnot glykémie. Dle pohlaví u dětí nebyl zjištěn významný rozdíl ve schopnosti sebepéče. Dále byl zjišťován vztah mezi délkou onemocnění a úrovní sebepéče, kde bylo prokázáno, že s celkovou úrovní sebepéče délka onemocnění nemá významnou spojitost, nicméně některé schopnosti lépe zvládají děti s delší délkou onemocnění. Toto zjištění se opět týkalo pouze schopností spojených s dávkováním inzulínu. Dále bylo zjištěno, že délka edukace nijak významně neovlivňuje sebepéči dětí. Závěr: Práce přináší další poznatky o schopnosti sebepéče dětí s diabetem, které mohou být přínosem jak pro děti, tak i pro jejich rodiny a celý tým zdravotníků, který o ně pečuje. Zjištěné výsledky mohou pomoci zejména edukačním sestrám, které na jejich základě mohou edukaci zdokonalit v oblastech sebepéče, ve kterých je to nejvíce nutné. Abstract:

ABSTRACT Aim: The aim of the diploma thesis was to ascertain the ability of self-care in children with the diabetes mellitus type 1. Furhtermore, selected factors influencing the ability of children´s self-care and the level of their knowledge in chosen areas have been recorded. Methodology: the examined group consisted of 101 patients of Children endocrinology department of the City hospital of Ostrava and Endokrinology and children diabetology surgery (doctor´s office) Medica Iberia, Ltd., in Opava, aged from 6 to 18 years. The questionnaire survey was carried out from 11th December 2017 to 11th February 2018, and a modificated Confidence in Diabetes Self-care Scale (CIDS) questionnaire was used. By this questionnaire, the ability of children´s self-care was examined. A descriptive statistics was used to describe the group. The Kruskal-Wallis test and the Spearman correlation coeficient were used to evaluate the hypotheses. Results: It was discovered that the overall level of self-care does not depend on a child´s age, hovewer, certain abilities and skills can be influenced by this factor. Children in the youngest age group cope worst with adjustments of insulin regime, and this ability improves with age. Children in the oldest group are the most confident when adjusting the insulin regime. On the contrary, the youngest children are the most conscientious in daily records of glycemic values. No significant difference in self-care was observed in dependence on child´s sex. Furhtermore, a correlation between the duration of disease and the level of self-care was examined, and it was proved that there is no significant link between the duration of disease and overall level of self-care, nevertheless, some skills tend to be mastered better by children with longer duration of illness. This finding again only concerns the abilities connected with insulin dosage. It was further discovered that the duration of education has no significant influence on child´s self-care. Conclusion: The thesis brings further findings about the self-care of children with diabetes, which can be beneficial both to children and their families, as well as the whole team of medical workers who take care of them. The findings can be especially helpful to educational nurses who can, on the basis of this information, improve the process of education in the areas of self-care where it is needed the most.
