Pro-thrombotic risk factors and their clinical significance in children in Czech Republic

Abstract:

Trombóza je u dětí stále vzácná, i když její výskyt v posledních letech narůstá, zejména se zvyšující se intenzitou léčby závažných onemocnění. Ve své práci pojednávám o významu vrozených trombofilních stavů v dětském věku. V souboru pacientů z ambulance Centra pro trombózu a hemostázu Oddělení dětské hematologie FN Brno, u kterých byly v letech 2005-2009 vyšetřena trombofilní rizika, bylo cílem zjistit důvody vyšetření, věk v době vyšetření, výskyt trombózy a frekvenci výskytu rizikových faktorů. Analýza dat reprezentativního vzorku dětí Jihomoravského kraje potvrdila obvyklé zastoupení jednotlivých rizikových faktorů, časté trombofilní mutace, vzácné deficity přirozených inhibitorů koagulace, jak u pacientů s trombózou, tak u ostatních vyšetřených dětí. Část dětí ze souboru byla vyšetřena v mladším věku, než odpovídá současným doporučením.V závěru uvádím doporučení pro praxi, kdy a v jakém rozsahu vyšetřovat trombofilní rizika u dětí.

Abstract:

Thrombosis is a rare in childhood, but its incidence has been increasing significantly in last 20 years. One of the main reasons is probably increased number and intensity of medical interventions in children during the treatment of various diseases. In my work I aimed to discuss the role of inherited thrombophilia risk factors for thrombosis development in children. I analysed data of children referred for thrombophilia screening to Thrombosis Haemostasis centre within Dpt. of Paediatric Haematology at Children's University Hospital Brno, Czech Republic in years 2005-09. I focused mainly on the reasons for the referral, on age, presence or absence of respective inherited risk factors (and thus their prevalence in Czech paediatric population) and presence/development of thrombosis. Analysis of the cohort showed usual distribution of inherited risk factors where thrombophilic mutations (e.g. F V Leiden, Prothrombin gene mutation) were more frequent than relatively rare deficiencies of natural coagulation inhibitors (AT, PC and PS). This was true for both patients with thrombosis as well as for asymptomatic children. Significant percentage of the paediatric patients were referred in younger age, than recommended. As mentioned above, the reasons for referral were also studied and evaluated. Based on the data obtained through my study I proposed recommendation when and whom to screen for thrombophilias during childhood in different clinical situations. Local institutional guidelines covering same have been developed as well.
