Abdominal Pain in Geriatric Patient from Paramedic's Perspective

Abstract:

ABSTRAKT HORÁK Josef. Bolesti břicha u geriatrického pacienta pohledem zdravotnického záchranáře. Vysoká škola zdravotnická, o.p.s. Stupeň kvalifikace: Bakalář (Bc.) Vedoucí práce: Mgr. Radomír Vlk. Praha 2023 66 s. Bakalářská práce se zaměřuje na vyšetření a správné postupy u geriatrických pacientů s bolestmi břicha v přednemocniční péči. Cílem bakalářské práce je zpracovat problematiku bolestí břicha u geriatrických pacientů v přednemocniční péči. Teoretická část se zabývá problematikou geriatrických pacientů, bolestí břicha, rozdělením břišních orgánů, možnými příčinami vzniku bolestí a patofyziologií. Dále se práce věnuje zdravotnické záchranné službě, problematice záchranné služby, typu posádek, stupňům naléhavosti a celkovému vyšetření pacienta v přednemocniční péči. Praktická část práce obsahuje čtyři kazuistiky. Prvním případem je muž, který trpěl od rána bolestmi břicha v epigastriu, svůj stav celý den neřešil, až odpoledne upadl do bezvědomí. Dalším případem je pád staršího muže z elektrokola, pád na zábradlí a následný úraz dutiny břišní. Třetí kazuistika se věnuje bolestem břicha postarší ženy, která byla vyšetřena ve zdravotnickém zařízení, odeslána domů, ale vlivem porušení léčebného režimu se u ní opět objevují bolesti. Poslední z kazuistik se věnuje postaršímu muži, který několik dnů pociťuje bolesti břicha v podbřišku, což ho vede k aktivování tísňové linky, ale následně odmítne transport do nemocnice. Součástí práce je i diskuze, která pojednává o správnosti postupů a celkové analýze, a závěr.

The bachelor's thesis focuses on an examination and correct procedures in pre-hospital care of geriatric patients with abdominal pain. The aim of the bachelor's thesis is to describe the issue of abdominal pain of geriatric patients in pre-hospital care. The theoretical part breaks down the issue of geriatric patients, abdominal pain, division of the abdominal organs, possible causes of pain and pathophysiology. Furthermore, the thesis is looking at the medical rescue service, the issues of the rescue service, the type of crews, the degrees of urgency and the overall examination of the patient in pre-hospital care. The practical part of the thesis contains four case studies. The first case is a man who suffered from stomach ache in the epigastrium since the morning. He did not pay attention to his condition all day until he fell unconscious in the afternoon. Another case is an elderly man who fell from an electric bicycle, the fall on the railing and a subsequent injury to the abdominal cavity. The third case report is focusing on the abdominal pain of an elderly woman who was examined in a medical facility and sent home, but due to a violation of the treatment regimen, pains started to appear again. The last case deals with an elderly man who has been experiencing abdominal pain in the lower abdomen for several days, that led him to calling on the emergency line and the subsequent refusal of the transport to the hospital. Thesis also contains a discussion, which breaks down the correctness of the procedures and the overall analysis, and a conclusion.
