The use of instrumental physical therapies as a supplement to the rehabilitation of a sports patient following L5-S1 discoplasty: clinical case

Abstract:

Intervertebral disc herniation, in particular in the lumbar part, represents the most frequent degenerative pathology of the spine in the population with a prevalence between 1% and 3%, a value that increases in subjects aged between 30 and 50 years, with a male / female ratio of 2: 1. Over 95% of lumbar disc herniations (LDH) occur at the L4-L5 or at the L5-S1 level. The main symptoms of LDH are low back pain and sciatica. In young athletes, low back pain is a common problem and the prevalence of back pain for various causes in the adolescent age group is between 20% and 30%, with an incidence that varies in different sports. The standard management of a patient with LDH, especially if he is an athlete, involves a first approach with conservative treatment with interruption of harmful activity and NSAIDs. In case of failure, surgery is opted for. In the clinical case treated, a 45-year-old male patient, employer, sportsman, with an LDH was faced with a conservative therapeutic approach, combining a physical treatment with rehabilitation. The treatment of the patient in question was divided into weeks with a first cycle of two weeks of physical therapy, followed by a combination of physical therapy and postural exercises in the following two weeks. After the first four weeks of treatment, the patient is re-evaluated and subjected to a new cycle that has been different from the first one. In the first week of the first cycle, it was decided to start with the TECAR therapy, in the first week of the second cycle, it was decided to use the laser therapy. In both cases, the physical treatment was accompanied by postural and stabilization exercises. Postural and stabilization exercises have been found to be effective in conservative treatment, reducing pain symptoms, improving quality of life, improving ROM and muscle strength. Furthermore, physical therapy has proved to be a good means of reducing painful symptoms, especially in acute pain, allowing the possibility of proceeding with the chosen rehabilitation program.

L'ernia del disco intervertebrale, in particolare quella che interessa il tratto lombare, rappresenta la pi? frequente patologia degenerativa della colonna vertebrale nella popolazione con una prevalenza compresa tra l'1% e il 3%, valore che aumenta nei soggetti di et? compresa tra 30 e 50 anni, con un rapporto maschi/femmine di 2:1. Oltre il 95% delle ernie discali lombari avviene a livello di L4-L5 o a livello di L5-S1. I sintomi principali dell'LDH sono la lombalgia e la sciatica. Nei giovani atleti la lombalgia ? un problema comune e la prevalenza del mal di schiena per cause diverse nella fascia di et? adolescenziale ? compresa tra il 20% e il 30%, con un'incidenza che varia nei diversi sport. La gestione standard di un soggetto con LDH, soprattutto se si tratta di un atleta, prevede un primo approccio con il trattamento conservativo con interruzione dell'attivit? dannosa e FANS. In caso di fallimento, si opta per l'intervento chirurgico. Nel caso clinico trattato, un soggetto maschio di 45 anni, impiegato, sportivo, affetto da LDH ? stato affrontato un approccio terapeutico di tipo conservativo, combinando il trattamento fisico a quello riabilitativo. Il trattamento del paziente in oggetto ? stato articolato in settimane con un primo ciclo per due settimane di terapia fisica, seguita nelle due settimane successive con una associazione di terapia fisica ed esercizi posturali. Dopo le prime quattro settimane di trattamento il paziente, viene rivaluto e sottoposto ad un nuovo ciclo diverso rispetto al primo. La prima settimana del primo ciclo si ? scelto di cominciare con la TECAR, nella prima settimana del secondo ciclo la laserterapia. In entrambi casi il trattamento fisico ? stato accompagnato da esercizi posturali e di stabilizzazione. Si ? visto che gli esercizi posturali e di stabilizzazione sono stati efficaci nel trattamento conservativo, riducendo la sintomatologia dolorosa, migliorando la qualit? di vita, migliorando la ROM e la forza muscolare. Inoltre la terapia fisica si ? dimostrata essere un buon mezzo per ridurre la sintomatologia dolorosa, soprattutto in acuto, consentendo la possibilit? di procedere con il programma riabilitativo prescelto.
