The role of omentin-1 in pathophysiology of complex diseases

Abstract:

Tuková tkáň produkující adipokiny je v současnosti chápána jako významný endokrinní orgán, jehož deregulace přispívá k rozvoji a průběhu metabolických onemocnění typu obezity, ale i komplikací spojených s těhotenstvím. Unikátní profily sekrece/exprese adipokinů dobře reflektují situaci v organismu a je možné je využít k hodnocení zdravotních rizik spojených se specifickou kumulací či úbytkem tukové tkáně. Disertační práce se zaměřuje na studium role nově popsaného adipokinu omentinu-1 v patofyziologii obezity a specifické porodnické nozologické jednotky (předčasných porodů), a to na základě hodnocení interindividuální variability v genu pro omentin-1 a hladin jeho proteinu. Tato disertační práce je koncipovaná jako soubor studií typu „případ-kontrola“, které se kromě meziskupinových srovnání hladin omentinu-1 zaměřují i na hodnocení vztahu těchto hladin s antropometrickými a nutričními parametry. Celkově bylo do studií zařazeno 557 subjektů, přičemž se jednalo o podskupinu obézních pacientů (index tělesné hmotnosti (BMI) ≥ 30), žen s předčasným porodem a žen s fyziologickou graviditou a porodem v termínu. Metodologicky byla pro detekci polymorfismů použita sekvenační analýza, polymerázová řetězová reakce (PCR) a restrikční analýza (RFLP). Pro kvantifikaci hladin omentinu-1 byla použita imunoenzymatická metoda ELISA. Analýza polymorfní oblasti rs2274907 (Val109Asp) v genu pro omentin-1 ukázala rozdíly v distribuci genotypů mezi obézní a morbidně obézní kohortou. Tato analýza dále prokázala signifikantně nižší průměrný denní energetický příjem a nižší příjem tuků a sacharidů při srovnatelném BMI u nositelů genotypu TT (Val/Val) v hodnocení celého souboru subjektů. S ohledem na detekci hladin omentinu-1 jsme pozorovali signifikantní negativní korelace omentinových indexů (poměru omentin-jiný adipokin, např. omentin-leptin) s vybranými antropometrickými parametry (váha, obvod boků, BMI). Dále jsme detekovali signifikantně nižší hladiny maternálního omentinu-1 v krevní plazmě u skupiny žen se spontánními předčasnými porody v porovnání se ženami, které rodily v termínu. Prokázali jsme potenciální vliv omentinu-1 na regulaci potravního chovaní a celkovou adipozitu (zmnožení tukové tkáně). Dále jsme potvrdili přínos použití hladin omentinu-1 v kombinaci s dalším adipokinem pro odhad antropometrických parametrů souvisejících s obezitou. Rovněž se domníváme, že omentin-1 je zapojen do patofyziologie předčasných porodů bez ohledu na to, zda došlo k předčasnému odtoku plodové vody či nikoli. Role omentinu-1 v patofyziologii komplexních onemocnění je v současné době intenzívně studována a zůstává předmětem dalšího výzkumu.

Abstract:

Adipokine-producing adipose tissue is currently perceived as an important endocrine organ whose deregulation contributes to the development and progression of metabolic diseases such as obesity as well as to complications associated with pregnancy. Since the unique secretion/ expression profiles of adipokines are well capable of reflecting the situation in the organism, they may be used to assess health risks associated with the specific accumulation or loss of adipose tissue. This thesis focuses on omentin-1, a recently described adipokine, and its role in the pathophysiology of obesity and preterm birth, a specific nosological entity, studied on the basis of an assessment of interindividual variability in the omentin-1 gene and the circulating levels of this adipokine. The thesis comprises a set of case-control studies focusing both on intergroup comparisons of omentin-1 levels and on the evaluation of the relationship between these levels and anthropometric and nutritional parameters in a cohort of 557 subjects including obese patients (Body Mass Index (BMI), ≥ 30), women with preterm birth and controls. In order to detect polymorphisms, the implemented methodological approach comprised sequencing, polymerase chain reaction (PCR) and restriction analysis (RFLP); circulating omentin-1 levels were quantified using the immunoenzymatic method ELISA. An analysis of the rs2274907 (Val109Asp) polymorphic region in omentin-1 gene revealed differences in genotype distribution between the obese and morbidly obese cohort and significantly lower average daily energy intake and lower fat and carbohydrate intake in TT (Val/Val) genotype carriers with comparable BMIs. With respect to omentin-1 levels, we observed significant correlations of omentin-1 indexes (ratio between omentin-1 and other adipokine, e.g. omentin-leptin) with selected anthropometric parameters (weight, hip circumference, BMI). We also described significantly lower plasma levels of maternal omentin-1 established in a group of women with spontaneous preterm birth in comparison to women who gave birth at term. We demonstrated the possible effect of omentin-1 on the regulation of eating behavior and overall adiposity while also confirming the benefit of utilizing omentin-1 levels in combination with another adipokine to estimate anthropometric parameters associated with obesity. We also believe that omentin-1 is involved in the pathophysiology of preterm birth regardless of preterm premature rupture of membranes. The role of omentin in the pathophysiology of complex diseases is currently under intensive investigation and remains the subject of further research efforts.
