Autoimmune diseases in data of NHIS of the Czech Republic as risk factors of subsequent diseases

Abstract:

Tato práce se zabývá hodnocením kauzálních vztahů autoimunitních onemocnění. Jejím hlavním cílem je podat informace o častých autoimunitních nemocí, jako je astma, celiakie, Crohnova choroba, diabetes, lupénka, roztroušená skleróza, Sjögrenova choroba a systémový lupus erythematosus a zkoumat jejich rizikovost pro získání nemocí dalších. Dále jsou definovány metody pro hodnocení vztahů jako spolehlivost, poměr šancí, podíl kauzální frakce, věkově nepřímo standardizovaná incidence a analýza přežití. Praktická část prezentuje charakteristiky datového souboru, který byl získán anonymizací a agregací dat Národního registru hrazených zdravotních služeb z let 2010–2018. Soubor obsahoval 6 992 903 pacientů, z nichž 1 501 952 trpělo autoimunitními nemocemi. Na tento soubor jsou poté aplikovány metody za účelem zjištění rizikovosti nastoupení dalších nemocí u pacientů s autoimunitními chorobami. Pomocí metrik poměru šancí, věkově nepřímo standardizované incidence a také podílu kauzální frakce bylo možné porovnávat nastoupení dalších chorob u pacientů s autoimunitními onemocněními s pacienty bez těchto onemocnění. Metrikou spolehlivosti byla zkoumána podmíněná pravděpodobnost nastoupení dalšího onemocnění, která nebyla zatížena nízkým počtem pozorovaných onemocnění. Bylo zjištěno, že pacientům s vybraným autoimunitním onemocnění hrozí riziko vzniku plicního a kardiovaskulárního onemocnění a jsou ohroženi vznikem alergické rýmy.

Abstract:

This thesis is focused on evaluation of the most common causal association autoimmune diseases. The main aim of this thesis was to inform about autoimmune disorders like asthma, celiac disease, Crohn's disease, diabetes, psoriasis, multiple sclerosis, Sjögren's disease and systemic lupus erythematosus and also examine their risk for having another illnesses. Then methods of causal association like confidence of market basket analysis, odds ratio, causal information fraction, standardized incidence ratio and survival analysis were defined. The practical part of the thesis like the characteristics of dataset, which was anonymized and aggregated from dataset of National registration of paid health services from 2010–2018, were presented. The dataset contained 6 992 903 patients, of whom 1 501 952 had the autoimmune diseases. Afterwards, the methods of causal association were applied to evaluate risk of occurrence another illnesses for patients with the autoimmune diseases. Using odds ratio, standardized incidence ratio, and causal fraction fraction was possible to compare the occurrence of another disease between patients with autoimmune diseases and patients without these diseases. The metric of confidence was used to examine the conditional probability of the occurrence another disease, that was not overestimated by the low number of observed diseases. Results have shown, that patients with selected autoimmune disease had higher risk of the occurrence of pulmonary and cardiovascular illnesses and they were threaten also by the occurrence of allergic rhinitis.
