Chromosomal aberrations in myeloproliferative syndromes

Abstract:

Problematika vzniku a vývoje myeloproliferativních onemocnění je to, čím se v mé práci zabývám. Tato skupina chorob je zapříčiněna různými mutacemi na genové úrovni. Práce pojednává o aberacích s nejvyšší frekvencí výskytu, kterými jsou Janusova kináza 2, kalretikulin, MPL a o fúzním onkogenu BCR-ABL. Hlavním cílem bylo provést mapování výskytu již zmiňovaných mutací. Součástí práce je popis celého procesu počínaje zpracováním vzorku, přes odběr biologického materiálu, až po popis použitých molekulárně genetických laboratorních metod. Použitou metodou byla zejména polymerázová řetězová reakce typu ARMS-PCR, kterou se prokazovala přítomnost mutací. Výsledky vyšetření byly získány z počítačového systému genetické laboratoře v nemocnici v českých budějovicích. Soubor obsahoval výsledky vyšetření u 302 pacientů za tříleté období, a to od roku 2017 do roku 2019. Data byla seřazena podle výskytu mutace a vložena do tabulek. Následně jsem provedla jejich součet a procentuální výpočet, čím jsem zjistila jejich dílčí výskyt. Na základě statistického vyhodnocení u jednotlivých mutací, jsem prokázala nejvyšší procentuální zastoupení mutace genu JAK2. Ta výrazně převyšovala nad mutacemi genů MPL a CALR. Závěrečné hodnoty aberací jsem pro objektivní posouzení porovnala s odbornou studií, která byla zaměřena na stejnou problematiku. Genetická vyšetření jsou dnes u těchto pacientů běžnou záležitostí. Začíná se uvažovat nad takzvaný genetickým profile pacienta v souvislosti s volbu léčby. Je možné, že na každý typ mutace, způsobující vážná onemocnění, fungují jiná terapeutika.

Abstract:

The issue of the origin and development of myeloproliferative diseases is what I deal with. This group of diseases is caused by various mutations at the gene level. The work deals with the aberrations with the highest frequency, which are Janus kinase 2, calreticulin, MPL and the fusion oncogene BCR-ABL. The main criterion was to map the expected mutations already mentioned. Part of the work is a description of the process, starting with sample processing, through the collection of biological material, to the description of data from molecular genetic laboratory methods. The method used was mainly polymerase chain reaction of the ARMS-PCR type, which proved the presence of mutations. Results of examinations of patients with a computer system of a genetic laboratory in a hospital in České Budějovice. The file contained the results of examinations in 302 patients over a three-year period, from 2017 to 2019. The data were entered according to the results of the mutation and entered into tables. Subsequently, I proved their sum and percentage calculation, I expected their partial occurrence. Based on the statistical evaluation of individual mutations, I proved the highest percentage of true JAK2 mutations. The one transferred over the mutations of the MPL and CALR genes. I am the final values of aberrations for objective assessment by evaluation professional studies, which were focused on insufficient issues. Genetic testing is a common concern in these patients today. The so-called genetic profile of the patient in connection with the possibility of treatment is being considered. It is possible that different therapeutics work for each type of mutation that causes a serious illness.
