Respect to human dignity of patients in coma in intensive care

Abstract:

Cíl: Cílem práce bylo zjistit, jak sestry pracující v intenzivní a resuscitační péči vnímají lidskou důstojnost, co si pod tímto pojmem představují, a zda respektují lidskou důstojnost pacientů v bezvědomí v praxi. Metodika: Provedený průzkum byl realizován formou kvantitativního výzkumu. Jako nástroj sběru dat byl použit dotazník vlastní konstrukce, který byl určen všeobecným a praktickým sestrám, pracujícím na odděleních intenzivní péče. Výzkumný soubor tvořilo celkem 87 respondentů. Průzkum probíhal v období ledna 2018 v Nemocnici ve Frýdku-Místku a v Městské nemocnici v Ostravě. Výsledky: Šetřením bylo zjištěno, že sestry v intenzivní péči ve většině případů respektují lidskou důstojnost pacientů v bezvědomí. Většina respondentů zkoumaného vzorku má konkrétní představu o pojmu lidské důstojnosti a dokáže jej charakterizovat. Sestry se chovají stejně k pacientům v bezvědomí jako k pacientům při vědomí, je pro ně důležité respektovat lidskou důstojnost pacientů v bezvědomí a považují veškeré informace o těchto pacientech za důvěrné. Za situace, kdy v praxi nejčastěji dochází k narušení lidské důstojnosti, sestry označily zajištění osobní hygieny, péči o vyprazdňování a zavedení permanentního močového katetru. Za příčinu nedůstojného chování zdravotníku respondenti považují nedostatek času personálu a syndrom vyhoření. Prokázána byla také nespokojenost sester v oblasti bazální stimulace a prostorového uspořádání pracovišť intenzivní péče, které bývá příčinou nedostatečného zajištění intimity a soukromí pacientů. Závěr: Problematika respektování lidské důstojnosti má u pacientů v bezvědomí zásadní význam. Tito pacienti se ve většině případů nacházejí v kritickému zdravotnímu stavu a nemohou se tak bránit případnému nedůstojnému zacházení.

Abstract:

Objectives: The aim of this thesis was to find out how nurses, working in intensive and resuscitation care department, perceive the human dignity. What do they construe under this concept and whether they respect the human dignity of unconscious patients in practice. Methodology: The evidence was obtained by means of quantitative research. As a tool for data collection was used a questionnaire of my own design. It was fulfilled by a general and practical nurses working in intensive care. A total of 87 respondents were surveyed. The survey was conducted in January 2018 at the Frýdek-Místek Hospital and the Municipal Hospital in Ostrava. Outcomes: The investigation has found, that nurses on intensive care department respect the human dignity of unconscious patients in most cases. Most respondents of the sample survey have a concrete idea of the concept of human dignity and they can characterize it. Nurses behave equally to unconscious patients as to conscious patients. It is important for them to respect the human dignity of unconscious patients and treat all information about these patients as confidential. Nurses have identified situations, where human dignity disruption often occurs. These are personal hygiene, care for defecation and installing a permanent urinary catheter. The lack of staff time and the burnout syndrome are considered by the respondents to be the cause of the nurses' inappropriate behavior to the patients. Also nurses' dissatisfaction with basal stimulation and the space arrangement of intensive care unit has been demonstrated to be the cause of insufficient grade of intimacy and patients privacy. Conclusion: The question of respect for human dignity is crucial for unconscious patients. These patients are, in most cases, in a critical health condition and can not hinder possible undisciplined treatment.
