Výskyt a léčba kostních změn u pacientů s chronickou pankreatitidou

Occurrence and therapy of bone changes in patients with chronic pancreatitis

Abstract:

Cílem naší práce bylo zjištění výskytu kostní demineralizace u pacientů s chronickou pankreatitidou, sledování vztahu funkčnosti pankreatické tkáně a etiologických faktorů k rozvoji osteopatie a kalciofosfátovému metabolismu a v neposlední řadě efekt zavedené terapie vitaminem D a kalciem. Prospektivně, po dobu 1 roku jsme sledovali 55 pacientů s endosonograficky diagnostikovanou chronickou pankreatitidou různé etiologie. Pacienti s jinou možnou příčinou osteopatie nebyli do souboru zařazeni. Při sledování kalciofosfátového metabolismu jsme využili stanovení jednotlivých biochemických parametrů a ve standardních oblastech jsme denzitometricky změřili kostní hmotu. U pacientů s prokázanou osteopatií jsme zahájili terapii kalciem a cholekalciferolem. U sledovaných pacientů se nám podařilo prokázat vysoké procento (43,7%) kostní demineralizace., ale vztah kostní demineralizace ke stupni pokročilosti CHP či operaci pankreatu se nám prokázat nepodařilo. Významný negativní vliv na kostní metabolismus má deficit vitaminu D, který je potencován přítomností pankreatické insuficience a dlouhodobým abusem alkoholu. Zavedení bazální substituční terapie vitaminem D a kalciem vede ke zvýšení sérových koncentrací vitaminu D a ke snížení procenta kostních demineralizací. Přestože u pacientů s pankreatickou insuficiencí je zmíněná terapie také efektivní a vede ke zlepšení parametrů kostního metabolismu, je vhodné zvážit způsob její aplikace, ve snaze co nejvíce potencovat terapeutický vliv na kostní hmotu. Vlivem onemocnění pankreatu na kostní hmotu se zabývalo jen velmi málo studií a proto zůstává tato oblast i nadále otevřena pro další sledování a výzkumy.

Abstract:

The aim of our work was to determine the incidence of bone demineralization in patiens with chronic pancreatitis, following the relation between the funcionality of the pancreatic tissue and etiological factors in the development of osteopathy and calciophosphate metabolism, as well as the effect of the therapy with vitamin D and calcium. Prospectivelly, during 1 year we followed 55 patients with chronic pancreatitis of different etiology verified by endoultrasound. Patiens with other possible cause of osteopathy were not included in the group. In the following of calciophosphate metabolism we determined different biochemical parameters and we measured the bone mass with densitometry in standard locations. In patiens in which the osteopathy was found we started the therapy with calcium and cholecalciferol. In the patients that we followed we managed to show high proportion (43.7%) of bone demineralization, however, no relation between the bone demineralization and the grade of chronic pancreatitis or the operation of pancreas was proved. Vitamin D deficiency has a significantly negative impact on bone metabolism, which is potentiated by pancreatic insufficiency and long-time alcohol abuse. The introduction of basal substitution therapy with vitamin D and calcium increases serum concentrations of vitamin D and a decreases the number of bone demineralizations. Although this therapy is effective also in patiens with pancreatic insufficiency, in which it improves the parameters of bone metabolism, it is advisable to evaluate the way of its application, in order to increase its therapeutic influence on bone mass as much as possible. Only few studies have dealt with the topic of influence of the diseases of the pancreas on the bone mass. Therefore, this part of medicine still remains opened for other followings and studies.
