Abstract:

Pojem polytrauma má ve společnosti stále větší význam. Z historických pramenů vyplývá, že polytraumata v minulosti vznikala v důsledku válečných konfliktů a dále s rozvojem průmyslu také jako pracovní úrazy. V druhé polovině dvacátého století, s rozvojem automobilismu, se přidružily dopravní úrazy. Pro diagnózu polytrauma je charakteristické závažné vícečetné poranění, které člověka bezprostředně ohrožuje na životě.Diplomová práce v části teoretické zpracovává poznatky k diagnóze polytrauma. Také se věnuje poznatkům o mechanismu vzniku úrazu, zaměřuje se na systémovou odpověď organismu ve vztahu k inzultu a také shrnuje poznatky z léčby. Výzkumná část diplomové práce je zaměřena empiricky a retrospektivně. Použitou výzkumnou metodou je kvantitativní výzkum se strukturovaným sběrem dat ošetřených polytraumatizovaných pacientů na Oddělení urgentního příjmu Traumacentra Fakultní nemocnice Olomouc v časovém úseku od 1. 1. 2010 do 31. 12. 2014. Data jsou zpracována prostřednictvím analýzy lékařskéa ošetřovatelské dokumentace. Hlavním cílem výzkumného šetření bylo zjistit, jaký je časový sled ošetření polytraumatizovaných pacientů v Traumatologickém centru Fakultní nemocnice Olomouc. Součástí hlavního cíle bylo uskutečnit šetření týkající se časového sledu diagnostiky a ošetření polytraumatizovaného pacienta z pohledu na přednemocniční neodkladnou péči a nemocniční neodkladnou péči. Dílčím cílem diplomové práce bylo provést šetření zaměřené na míru závažnosti polytraumat, ve vztahu k pohlaví a věku pacienta, za použití skórovacího systému Injury Severity Score. Výsledky výzkumného šetření budou použity pro zvýšení kvality poskytované péče pacientům se stanovenou diagnózou polytrauma na Oddělení urgentního příjmu Traumacentra Fakultní nemocnice Olomouc.

Abstract:

The term of polytrauma is becoming increasingly important in a society. From historical sources indicates that multiple traumas in the past were created as a result of armed conflicts and also with a development of industry as well as work-related accidents. In the second half of the twentieth century with the development of the car industry increased traffic accidents. For the diagnosis of polytrauma there are characteristic severe multiple injuries those are immediately man life-threatening.The theoretical part of the thesis processes with the findings about the diagnosis of polytrauma. It also deals with information about the mechanism of injury, focusing on the systemic response of the organism in relation to insult and it also summarizes findings from treatment. The research part of the thesis is focused empirically and retrospectively. The used research method is quantitative research with structured data collection of polytraumatised patients treated in the emergency Trauma department of the University Hospital Olomouc in a time interval from 1. 1. 2010 to 31. 12. 2014. Data are processed through the analysis of medical and nursing documentation.The main objective goal of the research was to find out what is the timing of treatment of polytraumatised patients in a Traumatology centre of the University Hospital Olomouc. Part of the main goals was to make a survey regarding the chronology of diagnosis and treatment of polytraumatised patient from the point of view of a hospital emergency care and department. Partial aim of this thesis was to perform an investigation of the severity of multiple traumas, in relation to sex and age of the patient, using a scoring system Injury Severity Score. The results of the survey will be used to improve the quality of care provided to patients with an established diagnosis of multiple trauma in the emergency department Trauma in the University Hospital Olomouc.
