Running biomechanics of athlete with a history of Achilles tendon rupture: Case Study

Abstract:

Ruptura Achillovy šlachy je devastující zranění postihující především aktivní jedince věnující se převážně raketovým sportům (tenis, squash), ale také postihující běžce. Cílem této práce je srovnat biomechaniku pohybu při běhu jedince s historií ruptury Achillovy šlachy s kontrolním jedincem bez zdravotních omezení. Odlišuje se tedy biomechanika pohybu při běhu jedince s historií ruptury Achillovy šlachy od biomechaniky pohybu při běhu od kontrolního jedince? Měřenými subjekty byli atlet ve věku 28 let, který podstoupil operaci Achillovy šlachy a rekreační sportovec ve věku 27 let bez poranění Achillovy šlachy. Měření jedinci byli párováni na základě podobnosti věku, tělesné výšky a hmotnosti. V této práci se zaměřujeme na interní abdukční moment v koleni, úhlu v hlezenním kloubu a vertikální reakční sílou výstupního výkonu v hlezenním kloubu. Probandi podstoupili měření běhu přes silové plošiny při rychlosti 3,2 m/s., na končetiny, trup a hlavu jim byly umístěny reflexní značky. Následné zpracování dat bylo provedeno v softwaru Qualysis Track Manager a Visual3D. Pro posouzení věcné významnosti byl použit index velikosti vlivu (ES) dle Cohena. Byl zjištěn vysoký interní abdukční moment způsobující riziko vzniku osteoartrózy, dále příliš nízká úroveň pronace, kdy dochází k poškození ochranného mechanizmu, a nakonec vysoká vertikální reakční síla, při níž vzniká riziko dalších běžeckých zranění. Jedinec s přetrženou Achillovou šlachou je vlivem zranění vystaven většímu riziku zranění při pravidelném běhu než jedinec kontrolní.

Abstract:

Achilles tendon rupture is a devastating injury affecting active people devoted to racket sports (tennis, squash) as well as runners. The aim of the thesis is to compare movement biomechanics during run of a person who suffered from Achilles Tendon rupture to a man with no health limitations. Does movement biomechanics during run of a person who suffered from Achilles Tendon rupture differ compared to a man with no health limitations? We measured the results in athlete aged 28 who underwent an Achilles Tendon surgery and a recreational athlete aged 27 with no health limitation. The athletes to be measured were chosen based on their similarity in age, height and weight. This work focuses on internal abduction moment in the knee, measured ankle joint angle and vertical reaction power output in ankle joint. Subjects were measured while running over power platforms at speed of 3.2 m/s. Reflection signs were placed on their limbs, trunk and head. We used Qualysis Track Manager and Visual3D softwares to process acquired data. We used Cohen Effect Size to verify subject importance. We measured high internal abduction moment causing an osteoporosis risk, followed by a low pronation level resulting in protective mechanism disturbance and finally high vertical reaction power bringing the risk of other injuries in runners. Subject after Achilles Tendon rupture is due to his injury exposed to greater risk to suffer injury during run compared to a man with no health limitations.
