Antimicrobial susceptibility testing of anaerobic bacteria, susceptibility profiles of anaerobes isolated in a University Hospital Brno in 2018

Abstract:

Anaerobní bakterie hrají významnou roli v mikrobiomu člověka. Jsou přirozenou součástí dutiny ústní, gastrointestinálního a urogenitálního traktu, ale mohou také způsobovat závažné infekce původu endogenního či exogenního. Společným rysem těchto bakterií je především anaerobní způsob života, tedy neschopnost bakterií růst za přístupu vzduchu. S tím souvisí i náročnost a odlišné postupy při odběru, zpracování či kultivaci vzorků. S kultivační náročností jsou též spjaty odlišné postupy stanovení citlivosti anaerobních mikroorganismů k antimikrobiálním látkám, které mají klíčovou roli v úspěšnosti ATB terapie. Vzhledem k tomu, že kultivace anaerobů je déletrvající a účinnost ATB je kvůli celosvětově rostoucí rezistenci čím dál více nepředvídatelná, je pro včasnou ATB terapii nutné znát profil citlivosti anaerobů v konkrétní oblasti a zdravotnickém zařízení. Práce má za cíl představit anaerobní bakterie, nastínit odlišnosti v diagnostice, a především popsat různé metody stanovení citlivosti k antibiotikům a poukázat na důležitost těchto metod pro klinickou praxi. Cílem praktické části je vyhodnotit profil citlivosti anaerobů k antibiotikům ve FN Brno a poskytnout tak užitečné informace pro klinické pracovníky při volbě empirické terapie.

Abstract:

Anaerobic bacteria play an important role in human microbiome. They are naturally present in the oral cavity, gastrointestinal and urogenital tracts but can cause serious infections of endogenous or exogenous origin. What is common to these bacteria is their primarily anaerobic way of life, i.e. the inability of the bacteria to grow in an environment where oxygen is present. This explains the difficulty and different procedures needed for sampling, processing or cultivation of samples. These demands also influence the procedures used for determining the susceptibility of anaerobic microorganisms to antimicrobials, which play a key role in the success of ATB therapy. Because anaerobic cultivation takes some time and the effectiveness of ATB is getting more unpredictable due to increasing global resistance, it is necessary to know the susceptibility profile of anaerobes in a specific area and hospitals for timely ATB therapy. This Thesis aims to introduce anaerobic bacteria, outline the differences in diagnosis, but above all describe the methods for determining susceptibility to antibiotics and point out the importance of these methods for clinical practice. The aim of the practical part is to evaluate the sensitivity profiles of anaerobes to antibiotics at the University Hospital Brno and thus provide useful information for clinicians regarding their choice of empirical therapy.
