Abstract:

Úvod: Onkologické onemocnění v dětském věku významně modifikuje potřeby dětí, preference jednotlivých potřeb a rovněž i jejich naplnění. Komplexní hodnocení potřeb dětí s onkologickým onemocněním je nedílnou součástí a prioritou ošetřovatelské péče. Kvalitní zhodnocení potřeb následně vede k efektivitě jednotlivých ošetřovatelských intervencí a především k prevenci vzniku komplikací onkologické léčby. V klinické ošetřovatelské praxi by se dětská sestra měla zaměřit nejen na hodnocení biologických potřeb, ale také na oblast psycho-sociálních a spirituálních potřeb. Pakliže chceme poskytovat kvalitní ošetřovatelskou péči, potřeby dětí s onkologickým onemocněním by měly být posuzovány komplexně. Cíl: Cílem práce je identifikace potřeb onkologicky nemocných dětí s akcentem na problematické oblasti potřeb souvisejících s onkologickou diagnózou a léčbou, včetně jejich saturace/naplnění. Metodika: Pro kvalitativní výzkum byla použita metoda ohniskových skupin (Focus group) a rozhovorů s dětmi s onkologickým nemocněním. Na základě analýzy dat kvalitativního výzkumu byly vyspecifikovány potřeby dětí, které sloužily jako podklad k vytvoření nestandardizovaného dotazníku. Kvantitativní výzkum byl proveden formou nestandardizovaného dotazníku, který mapoval biologické, psycho-sociální i spirituální potřeby. Výzkumný soubor byl tvořen 35 dětmi ve věku 15 - 18 let, které byly hospitalizovány ve vybraných zdravotnických zařízeních a podstupovaly onkologickou léčbu. Výsledky a diskuze: Byly stanoveny čtyři dílčí cíle a 15 výzkumných hypotéz. Výsledky šetření prokázaly nedostatky v identifikaci a saturaci potřeb dětí s onkologickým onemocněním ve všech dílčích oblastech potřeb. Jako nejvíce důležité považovaly děti vybrané potřeby: "Nemít potíže se zvracením", "Necítít nevolnost", "Nemít bolest", "Být ošetřován milým personálem", "Mít možnost setkat se se svými sourozenci". A jako nejméně naplněné se dětem jevily potřeby "Mít v budoucnosti děti", "Mít chutnou stravu", "Nebýt unavený", "Netrpět nechutenstvím", "Mít soukromí". Největší obavy měly děti z nauzey a zvracení. Zjištěné výsledky by měly vést ke zkvalitnění ošetřovatelské péče a především k uspokojování nalezených deficitních potřeb onkologicky nemocných dětí. Závěr: Byla zjištěna nedostatečná saturace ve všech dílčích oblastech potřeb. Posouzení potřeb dětí s onkologickým onemocněním by se mělo stát každodenní, nedílnou součástí ošetřovatelské péče. Dětská sestra by měla hodnotit komplexně veškeré biologické, psychosociální a také spirituální potřeby dítěte. Ošetřovatelská péče by měla být více zaměřena na systematické, kontinuální a komplexní hodnocení potřeb dětí s onkologickým onemocněním čímž by se zefektivnila saturace deficitních potřeb.

Abstract:

Introduction: Infantile oncological illnesses significantly modify the needs of children, preferences of individual needs as well as their fulfilment. A complex assessment of needs of children with an oncological illness is an integral part and a priority of health care. A quality assessment of needs leads subsequently to effectivity of individual nursing interventions and primarily to prevention of complications of an oncological therapy. In a clinical health care a children's nurse should focus not only on assessment of biological needs but also on the sphere of psycho-social and spiritual needs. If we want to provide a quality health care, needs of children with an oncological illness should be assessed in a complex way. Aim: The aim of this work is to identify needs of children with oncological illness, putting emphasis on the problematic area of needs related to diagnosis and oncological therapy, including their saturation/fulfilment. Methodology: Focus group method and interviews with children with cancer were used for qualitative research. Based on the analysis of qualitative research data, the needs of children were specified, which served as a basis for the creation of a non-standardized questionnaire. The quantitative research was conducted by the form of a non-standardized questionnaire, which mapped biological, psychosocial and spiritual needs. The research sample consisted of 35 children aged 15-18 who were hospitalized in selected health care facilities and were undergoing oncological treatment. Results and discussion: Four partial goals and 15 research hypotheses were set. The results of the investigation showed insufficiencies in the identification and saturation of needs of children with an oncological ilness in all subdomains of needs. The children considered the most important needs to be: "To have no vomiting problems", "To not feel sick", "To have no pain", "To be treated by nice staff", "To be able to meet their siblings". And the least fulfilled were the needs of "having children in the future", "having delicious food", "not being tired", "not suffering from appetite", "having privacy". Children were most worried about nausea and vomiting. The results should lead to the improvement of health care and, above all, mainly to the satisfaction of the discovered deficient needs of patients with an oncological ilness. Conclusion: Insufficient saturation was found in all subdomains of needs. Assessment of needs of children with an oncological ilness should become an integral everyday part of health care. The pediatric nurse should comprehensively assess all biological, psychosocial and spiritual needs of the child. Nursing care should be more focused on a systematic, continuous and comprehensive assessment of the needs of children with an oncological ilnesses, which makes saturation of deficient needs more effective.
