Nursing Process for a Patient with Psoriasis

Abstract:

Bakalářská práce se zabývá psoriázou a ošetřovatelským procesem pacientky, která byla s tímto onemocněním hospitalizována na metabolické jednotce intenzivní péče. Cílem práce je detailně popsat případ 60leté pacientky dlouhodobě trpící onemocněním lupénkou v akutním stavu. Je zde podrobně popisována péče o pacientku v průběhu tří dnů hospitalizace, stanovení ošetřovatelských cílů, plánu ošetřovatelské péče, jejich realizace a zajištění komfortu pacientky. Údaje o pacientce byly získány převážně z lékařské a ošetřovatelské dokumentace, v pozdější fázi také z primárního zdroje, tedy od samotné pacientky. Používané techniky k získání dat zahrnovaly rozhovor, pozorování a monitorovací techniky. K posouzení zdravotního stavu pacientky byl použit model Majory Gordonové. Na základě zjištěných a analyzovaných ošetřovatelských problému byly stanoveny ošetřovatelské diagnózy dle domén NANDA. U všech uvedených ošetřovatelských problémů byly uvedeny fáze ošetřovatelského procesu, a to sice plánování, realizace a hodnocení problému. Bylo stanoveno celkem osm ošetřovatelských problémů, z toho 5 aktuálních a 3 potenciální. Aktuální problémy zahrnovaly Akutní bolest (00132), Hypertermie (00007), Inkontinence stolice (00014), Neefektivní průchodnost dýchacích cest (00031) a Poškozená kožní integrita (00046). Jako potenciální problémy byly stanoveny Riziko aspirace (00039), Riziko infekce (00004) a Riziko imobilizačního syndromu (00040), což byly zásadní problémy pacientky v rámci ošetřovatelské péče a cílem bylo jejich zmírnění či předcházení. Onemocnění psoriázou představuje pro pacienty zdravotní, sociální i duševní problém. V případě neadekvátní či zanedbané léčby může vyústit v život ohrožující stav, kterému byla hospitalizovaná pacientka vystavena. V průběhu zpracovávání bakalářské práce byla pacientka přeložena na standardní lůžkové interní oddělení ve stabilizovaném stavu s nadějí navrácení do běžného života. Výsledkem bakalářské práce je doporučení pro praxi.

Abstract:

The bachelor thesis deals with psoriasis and the nursing proces of the patient hospitalized because of this disease in the intensive care unit of metabolic problems. The goals of the thesis was description of details of the 60 year female patient suffering from psoriasis for a long time and who became into acute phase of this disease. There are described important points of nursing care during three days of nursing care, like nursing goals, nursing plans, their implementation and provision of patient´s comfort during the hospitalization. The information about the patient was taken mainly from the medical and nursing documentation and also from the primary source becaue of communications with the patient in the later phase of patient´s hospitalization. The data was obtained by interview, observation and specialized monitoring techniques. The Majory Gordon model was used here for the patient´s health condition assessment. There were determined nursing diagnosis according to NANDA domains based on the identified and analyzed nursing problems. For all the mentioned problems there were given the phases of nursing process, namely planning, implementation and evaluation of the problems. There were described generally eight nursing problems, of which five were the actual and three were potentional. Current issues included Acute pain (00132), Hyperthermia (00007), Faecal incontinence (00014), Ineffective airway patency (00031), and Impaired skin integrity (00046). Risk of aspiration (00039), Risk of infection (00004) and Risk of immobilization syndrome (00040) were determined as potential problems. These problems are essential of patients in nursing care and the aim is to moderate or prevent them. Psoriasis is a health, social and mental problem for patients. If the treatment is inadequate or neglected, it can result in a life-threatenening condition to which the hospitalized patient was exposed. During the processing of the bachelor thesis, the patient was transferred to the standard inpatient internal ward in a stable condition with the hope of returning to normal life. The result of the bachelor thesis is a recommendation for practice.
