The proposal of standard of nursing care - Care of the arterial catheter

Abstract:

Cíl: Hlavním cílem bakalářské práce bylo vytvořit nový návrh ošetřovatelského standardu se zaměřením na ošetřovatelskou péči o arteriální katetr s implementací nejnovějších postupů pro využití v Nemocnici Nový Jičín. Tento vytvořený návrh poté předložit manažerům nemocnice ke schválení pro využití v praxi. Metodika: K vytvoření nového standardu ošetřovatelské péče byl získán souhlas vedení nemocnice. Poté bylo provedeno zmapování relevantních literárních zdrojů a požádány o rozhovor sestry z oddělení, které pracují s pacienty s nutností zajištění nebo zajištěným arteriálním přístupem, se zaměřením na informace o jejich praktických zkušenostech. K vlastnímu zpracování návrhu standardu byla využita nová šablona standardu ošetřovatelské péče, která je využívána v Nemocnici Nový Jičín pro tvorbu standardů ošetřovatelské péče. Výsledky: Z výsledků šetření vyplynulo, že některé ošetřovatelské postupy v péči o arteriální katetr, se liší od nejnovějších doporučených postupů a taktéž existují rozpory ve využívaných pomůckách, umístění tlakového převodníku, kalibraci systému nebo užití Heparinu v proplachové infuzi. Závěr: Na základě dostupné literatury a zmapovaných zkušeností sester byly do nového standardu zpracovány všechny nejasnosti, které vyplývaly z analýzy rozhovorů a navíc byly do získané šablony standardu zpracovány části: příprava personálu, příprava pomůcek a provedení převazu katetru, odstranění katetru a odběry z katetru. Abstract:

Goal: The chief goal of this bachelor's thesis was to create a new draft of the nursing standard, focusing on nursing care of the arterial line and implementation of the newest procedures for use at Nový Jičín Hospital. This created draft would then be submitted to the managers of the hospital for approval of use in practice. Methodology: The consent of the hospital's management was obtained in order to create a new nursing care standard. The relevant literary sources were then mapped and nurses from the department, where patients who require or have an existing arterial line are cared for, were interviewed with special focus on information about their practical experience. The new Nursing Care Standard template, which is used at Nový Jičín Hospital to create nursing care standards, was then used to execute the draft of the standard. Results: The results of the research indicated that some nursing procedures during care of the arterial line differ from the newest recommended procedures and that there are also discrepancies in the use of equipment, location of the pressure transducer, calibration of the system, or use of Heparin in the flushing infusion. Conclusion: On the basis of the available literature and mapping of the experience of nurses all unclear matters arising from analysis of interviews were clarified in the new standard and additional parts were also incorporated into the acquired template of the standard: preparation of staff, preparation of equipment and changing the catheter dressing, removal of the catheter and collection from the catheter.
