Stanovení profilu mastných kyselin v membránách erytrocytů nemocných s akutním koronárním syndromem

Determination of fatty acid composition of erytrocyte membranes in patients with acute coronary syndrome

Abstract:

Ischemická choroba srdeční s pokročilým zúžením koronáních tepen je obvykle léčena perkutánní koronární intervencí (PCI) s následnou implantací stentu. V reakci na implantovaný stent se však spouští zánětlivá reakce a intenzita zánětu souvisí s množstvím následných komplikací včetně restenózy. Zánětlivý proces navíc představuje hlavní složku v celé patogenezi aterosklerózy. Cílem této práce bylo zjistit, zda složení mastných kyselin v membránách erytrocytů, které odpovídá složení membrán kardiomyocytů, ovlivňuje stupeň zánětu a oxidačního stresu po implantaci stentu. Dvaceti pacientům se závažným zúžením koronárních tepen, kteří podstoupili PCI s následnou implantací stentu, byly odebrány tři vzorky krve, a to před PCI a dále po 24 a 48 hodinách. Ze vzorků byla získána plazma a erytrocyty. Profil mastných kyselin v membránách erytrocytů byl analyzován pomocí plynové chromatografie. V plazmě byl změřen marker lipoperoxidace (malondialdehyd, MDA) a na Oddělení klinické biochemie a diagnostiky Pardubické krajské nemocnice byl v plazmě změřen marker zánětu (C-reaktivní protein, hsCRP). Na základě dynamiky těchto markerů byl zaznamenán vzestup zánětlivé odpovědi i oxidačního stresu s maximem 48 hodin po PCI. Při hodnocení vztahu mastných kyselin k zánětu bylo zjištěno, že kyselina olejová a aktivita ?9-desaturasy zánětlivou reakci snižují. Dále byla prokázána negativní korelace kyseliny cis-palmitolejové a v menší míře také kyseliny olejové s navýšením oxidačního stresu po PCI. Pozitivní korelaci s navýšením oxidačního stresu po PCI vykazovaly kyselina eikosapentaenová a dokosahexaenová. Dlouhodobé působení mastných kyselin na průběh aterosklerózy bylo zjišťováno porovnáním procentuálního zastoupení mastných kyselin v membránách erytrocytů u pacientů a kontrolní skupiny zdravých dárců krve. Pacienti měli oproti kontrolní skupině významně vyšší zastoupení prozánětlivých kyselin palmitové a stearové i celkového množství nasycených mastných kyselin. Procentuální zastoupení kyseliny cis-palmitolejové, bylo signifikantně vyšší u kontrolní skupiny. Hodnoty n-3 PUFA byly velmi nízké u pacientů i u kontrolní skupiny, což vypovídá o obecně nízkém dietním příjmu těchto kyselin v naší populaci.

Abstract:

Ischemic heart disease with advanced coronary artery stenosis is usually treated with percutaneous coronary intervention (PCI) followed by a stent implantation. In response to the implanted stent the body triggers an inflammatory reaction and the intensity of inflammation is directly related to the amount of subsequent complications, this includes restenosis. The inflammatory process is a major component in the pathogenesis of atherosclerosis. The aim of this study was to determine whether the composition of fatty acids (FA) in erythrocyte membranes, which corresponds to the composition of the membranes of cardiomyocytes, influences the degree of inflammation and oxidative stress after stent implantation. Prior to PCI, 24 hours and 48 hours later three blood samples were taken from twenty patients with coronary artery stenosis undergoing PCI with subsequent stent implantation. Samples were obtained from plasma and erythrocytes. Fatty acid profile in erythrocyte membranes was analyzed by gas chromatography. Marker of lipid peroxidation (malondialdehyde, MDA) was measured in plasma and at the Department of Clinical Biochemistry in Hospital of Pardubice was in plasma measured marker of inflammation (C-reactive protein, hsCRP). Based on the dynamics of these markers an increase was observed to the inflammatory response and to the oxidative stress intensity within a maximum of 48 hours after PCI. When evaluating the relationship of FA to the inflammation it was found that the oleic acid and the activity of ?9-desaturases reduce the inflammatory response. Furthermore, a negative correlation of the cis-palmitoleic acid was proven and to a lesser extent of the oleic acid with increased oxidative stress after PCI. Eicosapentaenoic acid and docosahexaenoic acid showed a positive correlation with the increase in oxidative stress after PCI. Long term effects of fatty acids on the progression of atherosclerosis was determined by comparing the percentage of FA in erythrocyte membranes of patients and a control group of healthy blood donors. The patients in comparison to the control group had a significantly higher proportion of pro-inflammatory palmitic acid, stearic acid and the total sum of saturated FA. The percentage of cis-palmitoleic acid was significantly higher in the control group. Values of n-3 PUFA were very low for both patients and the control group which indicates a generally low dietary intake of these acids in our population.
