Importance of diuresis and urine analysis for diagnostics of renal and metabolic disorders in experimental sepsis and critical illeness

Abstract:

Ledviny hrají významnou roli v udržování homeostázy vnitřního prostředí. Akutní poškození ledvin (acute kidney injury, AKI) představuje závažnou komplikaci v intenzivní péči spojenou s vysokou mortalitou. První část této práce obsahuje teoretický úvod k AKI, anatomické a fyziologické poznámky důležité pro funkci ledvin a jejich poruchy. Historie pojmu akutního renálního selhání je krátce shrnuta výčtem prací, které jako první pojmenovaly a popsaly toto postižení. Definice pojmu je pojata z hlediska dostupných parametrů, pomocí kterých jsme schopni AKI zachytit a popsat. Dále je uvedena kritika těchto parametrů. Podrobně je v práci popsána nejaktuálnější klasifikace postižení ledvin, tzv. RIFLE klasifikace. (R – risk, riziko, I – injury, poškození, F – failure, selhání L – loss, ztráta, E - end stage, konečné stadium). Kapitola patofyziologie renálního poškození je rozdělena na dvě části: poruchy glomerulární filtrace a poruchy proximálního a distálního tubulu a Henleovy kličky. V další kapitole je shrnuta epidemiologie onemocnění a její sledování v posledních deseti letech. Pojednání o léčbě je rozděleno na léčbu konzervativní a léčbu pomocí náhrady funkce ledvin. Dále jsou probrány látky, které mohou vést k poškození ledvin u kriticky nemocných. Druhá část práce je věnovaná vlastnímu výzkumu, zaměřenému na sledování diurézy a na analýzu moči u různých stavů v intenzivní péči. Tvoří jej čtyři studie. Cílem první, prospektivní experimentální studie bylo sledování a porovnání změn acidobazické rovnováhy, iontů a osmolality v krvi a moči během prvních 12 hodin u experimentálního modelu sepse. Zjistili jsme, že časné biochemické změny v krvi jsou v řádu hodin následovány změnami v moči. Změny v moči jsou výraznější než změny v krvi. Ve druhé studii byla sledována incidence intraabdominální hypertenze a hypoperfúze splanchnických orgánů po velkých nitrobřišních výkonech. Jednalo se o observační prospektivní studii, sledující přínos sledování nitrobřišního tlaku na chirurgické jednotce intenzivní péče. V práci došlo však jen vzácně ke zvýšení nitrobřišního tlaku. Podobně hypoperfúze žaludeční sliznice a hypoperfúze jater či pokles diurézy byly ojedinělým nálezem. Třetí, prospektivní klinická studie sledovala, jak nemocní s vyjádřenou celkovou či lokální metabolickou acidózou reagují na podání standardní dávky bikarbonátu. Zda bikarbonát „retinují“, a tudíž nedojde k alkalizaci moče, či ne. Přínosem by pak byla časnější a přesnější možnost záchytu metabolické poruchy. U pacientů s metabolickou acidózou však tato reakce nebyla jednoznačná. Čtvrtá studie zjišťovala, zda sledování diurézy zvyšuje incidenci akutního renálního selhání u nemocných, vyžadujících dlouhodobou intenzivní péči. Jednalo se o retrospektivní analýzu chorobopisů. Práce souvisela s rozšířením a změnou definice renálního postižení o diurézu a vedla k vyšší frekvenci jeho záchytu.

Abstract:

Kidneys play an important role in maintaining internal environment homoeostasis. The acute kidney injury (AKI) is broadly defined as deterioration of renal function. It often occurs in intensive care units and is connected with high mortality. The first part contains a theoretical introduction to AKI and anatomical and physiological comments significant for renal function and diseases. The history of the term “AKI” has been summarized by outlining important papers which first described this disease. The definition of the term has been viewed in the light of available parameters that help us record and describe AKI, including comments on them. The paper describes the most topical classification of renal failure, so called the RIFLE criteria (R - risk, I - injury, F – failure, L – loss, E – end stage). The chapter on pathophysiology of renal disease is divided into two parts: Defects of glomerular filtration and Failures of proximal and distal tubules and the loop of Henle. The following chapter outlines the epidemiology of AKI and its monitoring over the last ten years. The part relating to treatment is divided into conservative treatment and the treatment with renal replacement therapy. Also, substances with nephrotoxicity potential are mentioned. The second part of this work contains the actual research focusing on diuresis observation and analysis of urine in different situations in intensive care. It contains four studies. The first study was prospective and experimental. It monitored and compared some changes in acid-base balance, ions and osmolality in blood and urine during the first 12-hour phase in an experimental model of sepsis. We have found out that the early biochemical changes in blood were followed within several hours by early biochemical changes in urine. The urine changes are described in more detail than those in blood. The second study observes the incidence of intraabdominal hypertension and splanchnic hypoperfusion after major abdominal surgery. This was an observation-based prospective study focusing on the benefits of intraabdominal pressure monitoring in intensive care units. However, patients after uncomplicated major abdominal surgery seldom experience clinically significant IAP increase. Similarly, gastric mucosa hypoperfusion, liver hypoperfusion and decreased diuresis were also very rare. The third prospective clinical study investigates how patients with manifest or local (splanchnic) metabolic acidosis (MAC) respond to administering the standard dosage of bicarbonate - if the bicarbonate is retained (thus urine alkalinization does not occur) or not. The benefit would be a possibility of earlier and more precise diagnosis of metabolic failure. However, the response was not clear in the patients with metabolic acidosis. In this complex situation, it is not possible to confirm our hypothesis. The fourth study examined if diuresis monitoring increases the incidence of the AKI in long-term ICU patients. This retrospective analysis of patients´ case records was related to an expansion and a change of the definition of the AKI (inclusion of diuresis), which lead to an increased frequency of its diagnosis.
