Abstract:

Předkládaná diplomová práce se zabývá vlivem terapie v přístroji Lokomat na posturální stabilitu u pacientů s roztroušenou sklerózou. V teoretické části je popsána roztroušená skleróza a samotný systém Lokomat, v kterém bude terapie probíhat. Výzkumný soubor tvořil 30 pacientek s diagnostikovanou RS. Délka trvání nemoci byla v rozmezí 5 až 15 let a dle Kurtzkeho škály stupeň 4 až 6. Celá praktická část diplomové práce probíhala ve Fakultní nemocnice Ostrava na Klinice rehabilitace a tělovýchovného lékařství. Probandi byli osloveni k účasti v rámci programu rehabilitace u RS, který probíhá také ve Fakultní nemocnici Ostrava na neurologické klinice. Na FreeStep plošině byly hodnoceny parametry: sway, výstřednost, maximální výchylka a celková plocha elipsy. Z klinických testů byly vybrány UpandGo a Four Square Step Test. Jak experimentální, tak kontrolní skupina absolvovala dvouměsíční rehabilitační program, který zahrnoval skupinová cvičení. Experimentální skupině bylo zařazeno navíc 5 až 8 terapií v přístroji Lokomat. Z výsledků práce je viditelné, že po terapii v přístroji Lokomat došlo ke zlepšení parametrů posturální stability. V klinických testech došlo ke statisticky významnému zlepšení oproti kontrolní skupině.

Abstract:

The presented diploma thesis deals with the effect of therapy in the Lokomat device on postural stability in patients with multiple sclerosis. The theoretical part describes multiple sclerosis and the Lokomat system itself, in which the therapy will take place. The research group consisted of 30 patients diagnosed with MS. The duration of the disease was in the range of 5 to 15 years and according to the Kurtzke scale grade 4 to 6. The entire practical part of the thesis took place in the Ostrava University Hospital at the Clinic of Rehabilitation and Physical Education Medicine. Probands were approached as part of the multiple sclerosis program, which also runs at the Ostrava University Hospital in the neuralgia clinic. The following parameters were evaluated on the FreeStep platform: sway, eccentricity, maximum deflection and the total area of the ellipse. UpandGo and Four Square Step Test were selected from the clinical tests. Both the experimental and control groups completed a two-month rehabilitation program that included group exercises. In addition, 5 to 8 therapies in the Lokomat device were included in the experimental group. From the results of the work, it can be seen that postural stability parameters improved after therapy in the Lokomat device. In clinical tests, there was a statistically significant improvement over the control group.
